The song is actually describing Hughes later in life, when he was holed up in a Las Vegas hotel, with all the windows blacked out, not seeing anyone, letting his hair and fingernails grow, and keeping his urine in jars, suffering from chronic pain, obsessive compulsive disorder (OCD), and acute paranoia.
