      regarding your editing. I was impressed by how quickly you covered all the material.
      it was 1:50 at the beginning of the video - you were able to squeeze it all in by 1:50. Either very very very fast or the filming took 12 hours i.e. 1:50pm to 1:50am. ya I know, green screen

      Surgeries were on L5 and S1. I've had steroid shots and epidural, which help with pain. I have weakness, I stumble and sometimes fall.

      I been told i hv a pinch nerve call disk arnia c6 c7 i might need surgery . Physio mentioned to me to be careful wt neck surgery i can bw paralyzed. Now im more confusing

    L5 S1 radiculopathy here. I'm on week , 15 of PT. Pain in left foot only getting worse. Begging for shots or oral steroids. No NSAIDs allowed. Back issues for 50+ years. Best presentation I have ever seen

    I have spinal stenosis in L4and L5,diagnosed in 2018. Was consulted to a neurosurgeon by my neurologist but unfortunately up to now haven’t heard from him. It’s getting worse in fact I am using a walking cane as sometimes sudden buckling of my leg. I was finally seen by a pain specialist. With the treatment with medications it’s helping and am using a lumbar belt. The fortunate part of it is my right leg is good with no pain or any other symptoms and I can drive a car. By the way I also have seen a physiotherapist and am doing the exercises that was told to do it bid. Your video is quite excellent and I can fully understand my situation . Thank you.

    As a primary care physician of 25 years and fellow disc disease and spinal stenosis sufferer, I can attest to everything Dr. Donnally is saying as spot on 100%. I'm pleased to see that this is how I've been advising my patients. Surgery in many cases should be tried only when all else fails or if time fails to resolve the problem.

    I have recently had x-rays and an MRI, both indicating lumbar spinal stenosis as well as disc degeneration. I have been trying to translate all the medical terminology used in the MRI report so I can understand the true situation.
    This video is excellent in explaining the various elements involved, the possible conditions and how they can affect each other, and various procedures - primarily non-surgical - to help relieve pain.
    Thank you, Dr. Donnally, for your clear, concise talk in lay terms so we non-medical folks can grasp and understand the body structure and elements involved, what can go wrong, and how different procedures work to counteract the problems.

    Thank you so much for explaining spinal stenosis so clearly. Just to add to what you’ve addressed. I had two pain management treatments under sedation. They failed. Tried it one more time - no sedation and it worked perfectly….no pain. The difference was I could fee the doctor’s manipulations….you do get a local so there’s no pain. The doctor would ask “can you feel it now …left or right?”. We continued this until I couldn’t feel any pain left or right.😊

    71 year old athlete in Tulsa. Sprinted in yard and tripped Jan 6. Many visits to 3 orthopedic surgeons and 2 MRI revealed torn hamstring and an extruded herniated disc in the L-5, S-1. Two months of excruciating sleep-depriving constant pain. My Primary Care DO referred me to the Pain Management Center. Yesterday morning while prepping for Epidural, the techs mentioned that "sometimes" if no relief people will schedule another treatment the next week. During procedure, the doc and her female techs maintained what seemed a super casual conversation about plans for St Paddy's day while I was excruciating for the few minutes of treatment. You mentioned at 10:24 "unethica pain management. When a tech plants the notion of another treatment the next week my red flag goes up. Your presentation is the best!!

    Had a MRI years ago, was sent to a doctor who does spinal blocks, he would not do one on me, said he didn't want to paralyze me. I am doing therapy now helps me straighten up, relieves some pain. Had back problems since I was 36, am 76 now. I always have pain, have learned to live with it. I like standing up straight, I don't feel like everyone stares at me.

    Great information Doc. I'm a retired nurse. I went to pain management doc here in Tucson. The injections have greatly decreased my pain
    I have had two spinal surgeries. I have a fusion of S 3,4, and S1. Currently, I'm having aggressive Pt. I'm walking and have a deep appreciation of you caring docs.

    This is the third video from other professionals I've listened to about lumbar spinal stenosis and I have to say Dr. Donnally convinced me to get the injection of steroids that my pain management doctor wants to give me. My back pain is between L1-L5 and has affected my quality of life so much that I cannot stand more than 4 minutes or walk more than 8 minutes without the pain nearly causing me to crumple. Dr. Donnally has a very good bedside manner with a clear voice, specific pronunciation and my guess would be very kind to his patient. Even though I have so much pain in my back you would think a needle wouldn't scare me but it does because I am deathly afraid of the pain the needle will cause. I have bookmarked this video so I can watch it again. Thank you Dr. Donnally.

My pain management doctor offered to prescribe two Xanex to take before the appointment. Hopefully I'll work up the nerve to get the injection. Thank you for responding.

All the doctors I spoke to and information I have read say that the patient needs to be awake to be able to answer questions the doctor will ask such as do they feel pain in their legs or feet. If they do that is bad and the doctor will either stop or put the needle in a different spot.

    Thank you for the clear and definitive explanation. EVERYTHING you discuss in this video had happened to me. It started with scoliosis, then enherniated discs, now severe, high-grade spinal stenosis. I have 3 bulging discs, spinal stenosis, and now the surgeon that I consulted in Georgetown says that without the cartilage, I have an upper disc that has permanently slipped below the lower one. He showed me the MRI and explained that at the lower discs, the nerves are not visible at all because of the severe compression. Not to mention degenerative disc disease. My conditions are also complicated by tearing both of my hamstrings and they didn't heal properly. My primary care doctor reviewed the MRI and said "WOW, that's an awful lot going on with your back!" No kidding. The surgeon said that he couldn't believe that I could still walk because I had the worst case he's ever seen. He wanted to do surgery, but a 3-month recovery with NO bending, lifting or twisting. Since I live alone, that's not happening. The constant excruciating pain has become so unbearable that I can't even sleep in bed anymore, but I have to sit up on my couch. On some days, it's tolerable. It's also exacerbated my chronic sleep disorders. I'm just a mess in general. I'm in Killeen.

      I understand. The doctor that looked at my mri said he had not see one person that has so much bad going on in their back. I had one day this past week where I had hours without pain. It was wonderful. I try not to get depressed. Cold weather really bothers me. If you are a praying person we could pray for each other. ❤

    I was diagnosed 5-6 years ago with spinal stenosis, with two bulging discs around l4 and l5. I have had subsequent years of leg (hams & quads) muscle atrophy and twitching, and pain. I am finally going to a surgeon soon. Thank you for your thorough and accessible video.

    Thank you for all you do to the community Dr D. Suffered from left leg sciatica pain for a year and a half and this year I changed my insurance to PPO which greatly improved my chances of seeing the doctors that I think are best for me. PT, injections and oral meds didn't help. Went to Dr #1 and he recommended L4L5 disectomy, laminectomy and all the works. But I thought I have 3 discs slipping etc etc and why not do more since he's going to open me up anyways.
    Went to Dr #2 from one of Chicago's top ortho hospital (with your encouragement) and #2 did additional L4L5 fusion and pain was totally gone right after surgery. I'm thankful to you for calming my nerves before undergoing surgery. Now I'm totally pain free thank God!

    BRILLIANT! Probably the best shortcut to learn and understand radiological terminology and interpretation of MRI scan on your lumbar spine..
    Been there, done it , avoided the surgery by the skin of the teeth ! ..and defied a top London surgeon's prognosis !

    Wow! That was an excellent presentation. I was a career Army officer and we always tried to comminicate very complex things 'at the 8th grade level' so everyone can understand the task. In order to do this you have to really know your topic. I think an 8th grader could understand your presentation. I'm going through the lumbar/sciatica process now. My chiropractor recommended I persue the pain injections. He mentioned the diagnostic benefit as well. I have found the injections very helpful for my neck. At the VA where I get treated, for a chronic problem you have to be referred to the spine surgeons by pain management or physical medicine, so they don't QB the injections.

    Thank you for all this information. I have had very bad pain in lumbar area since 2007, most likely due to long time acromegaly. I am quite concerned (terrified) to even consider surgery. I really appreciate your clear explanations. I have had several surgeries including pituitary tumor resection, hip and knee replacement, but spine surgery is the scariest. After seeing your video I am again going to consider it. Thank you.

    I was just diagnosed with spinal stenosis this week and as I lay here in bed with my leg in pain, I came across this video because I could not sleep. This is a really great video explanation in all areas. I want to avoid surgery as much as possible. I am due to get an MRI. I have also been given an order for physical therapy so will be starting that soon. thank you for the time that you put into making this video for those of us that don’t understand the medical terminology I could understand this.

    This is the best information I have ever received. I have had severe lumber and cervical spine issues with pain since I was ten years old. Car accident trama. I'm now 63 years old.
    I have had three lumbar surgeries in the L5-S1 area. 1999, 2001, and 2004. The only one the truly helped was in 2004. All of the surgeries were for removal of fragmented disks.
    Now that the pain in the lumbar area is so severe, after multiple injections, I was advised that I need to have the disks removed and spacers placed there for a fusion.
    The cervical area in just has bad and needs the same treatment.
    I sure wish I lived in Texas could be treated by you.

    Both of my feet are numb apparently from spinal stenosis but am not a candidate for surgical intervention due to "poor bone quality", ie, osteoporosis. I'm kind of backed into a corner 😁 I appreciate the thorough info presented so I can discuss this with my Dr with more understanding of my DX 👍

    I am currently doing physical therapy for lumbar stenosis. I cannot stand for more than 10-15 minutes before lower back pain is pretty severe. The pain is changing areas, from the lower back to buttocks and down the back to my knees.

    This is such a great explanation of lumbar spinal stenosis, thank you so much! It took more than 10 years for me to get a diagnosis and I managed things for several years with rehab exercises, but it has flared up this year and progressed to tingling and numbness in legs and feet. I'm trying to get a consult with a surgeon here in Toronto. I had a cortisone injection 5 weeks ago and it helped with some pain but did nothing to alleviate the numbness. I can barely walk 100m, if it's a good day, before my feet go numb and sometimes it feels like my legs are about to buckle. At least I feel better informed about what may actually be happening.     That was an excellent explaining, thank you so much. I am going to be seeing pain management for the epidural injections of L4-5 and L5-S1. I have several levels of degenerative disc disease and disc desiccation too.
    My question is how long can I continue to take anti inflammatories like Aleve, or Advil without it causing harm to my liver or stomach??
    I’ve been taking a combination of Advil and Tylenol alternating every 4 hours. (Doing this until I can get my epidural scheduled). I also realize I will need to stop the advil before the procedure. I have also had several courses of medrol dosepack over the last 8-9 weeks because of the pain. Fortunately my pain has begun to subside slightly, and I am not showing weakness in my legs any longer. But that’s only been about a week or so now. I hate taking the meds but the pain was debilitating at first. So glad it is beginning to get better, or maybe the meds are just working better now.

    I have this issue with weak muscles due to nerve degeneration. Is there any way that nerves might heal after surgery. It has been over twenty years when I was diagnosed with it and I opted not to go for surgery.
    Would appreciate a reply.

    I had Spondylolisthesis from pars fractures that formed between ages eight and fifteen. That and arthritis and disc herniation caused my stenosis was so severe (bone on bone) where I had no choice but to have anterior and posterior spinal fusion surgery and that really helped me. The Spondylolisthesis and Stenosis is also genetic.

    Very interesting information. I have been suffering with lower back pain for the past four years but now it’s getting better because I exercise regularly. Sometimes I get this shooting pain right down my leg and it’s very painful. I was diagnosed with lumber disc herniation of the third and fourth disc. What else must I do to get rid of this situation?     Awesome video! I had surgery for spinal stenosis 3 years ago. They didn’t fuse any vertebrae but rather removed whatever was causing the stenosis. The pain in my legs was completely gone but has recently returned. The surgeon told me that he will install plates and screws next time which I would like to avoid. I’m going to pt now hoping I can get some relief. It seems to be helping. I’m gonna ask my doctor about the steroids and injections.

    I have the weakness in legs - especially the right leg. PT tried to get me to increase strength, but it caused more back problems. Been in PT for years.
    Just discovered many of the exercises in PT could be making spinal stenosis worse. Those are lower back strengthening exercises - like bird dog, or superman, hip flexor exercises - like leg raises or marching, and piriformis stretching, or reaching across to feet while sitting and hamstring stretches.
    I've got to get the stenosis specific exercises, like abdominal bracing, pelvic tilt while holdingback and abs down and contracting glutes, knees to chest
    while holding the back down and glutes engaged, angry cat and ab curls - both withkab tightening and glutes engaged. Are there any PT stenosis specialists out there? All of mine seem to want me to do other stuff that aggravate my condition - while telling me that I may need hip replacement.

      @@dealerspot Probably from claudation of the central spine at L3. There may be lateral stenosis causing it as well as disc degeneration in the lumbar region. I've been using a cane lately and may have to switch to a walker just to keep the pressure off bilaterally.

    I'm 33, and recently was rushed to emergency to be told i have this. I had supposed it was a hamstring issue, because of the pain in my left thigh and sciatica. Turns out thats just the Symptom. But I'm partially bedridden because of the pain. Surgery was mentioned to me but I'm on medication instead. It doesn't give me 100% mobility but it helps with the pain.
    These videos are very informative and have helped me a lot. Thank you.
    I don't know what my next step is.
    I just wanna walk again.

      Read the black box warnings before the injection also look up and study arachnoid adhesions. Scarring where injections placed. Germany has many options that keep the spine mobile and strong.

    Informative....a little fast for the patient with no knowledge of the issues. I've been dealing with L4L5 stenosis for several years now. I have no life! Having surgery on 4/6/23 in Tucson. My doctor is Travis Dumont. He's kind, confident, and well trained! Minimally invasive procedure. Wish me luck!

    In December 2017, I was diagnosed with spinal stenosis of the L4-5 due to congenital narrowing of the spinal canal, scoliosis, and degenerative arthritis! I had no pain whatsoever, but would have extended episodes where I couldn't feel my legs from the waist down. I felt like a floating torso. LOL. I had a laminectomy and spinal fusion of L4-5 and was 100% better. (I had tried PT for a couple of months, but it made it worse.) Thank you for this teaching. It helps me going forward.

    Dr Donnally - Have been to MRI twice (2015 and 2018) showing SDDD in lumbar area. Am 79 yo. Had almost two years of PT which resulted in increased discomfort and discontinued it. Have had CABS (quadruple bypass October 2020 ). Lumbar symptoms began in 2015 with difficulty stepping into my pants with weak legs. Had occasional backaches without nerve pain. Had compression lumbar injury in 1969 auto accident. Used to work out at gym from 1988 to about 2014 which kept me in great shape and alleviated lumbar back pain. Got bitten by a tick around 2010, got the bull's eye rash and eventually got connective tissue disease from Lyme disease, resulting in popped shoulder tendons and loss of much arm range of motion. For about two years was treated by a Lyme doctor with no success from antibiotics. From about 2016 started with severe body/leg weakness after 1/2 hour or so after getting up in the morning. If I lay down for an hour or so my body would 'heal' up so that I could walk again for another half hour. I used to do line dancing until 2017 when leg weakness caused that to end. Now it is very strenuous and weakeningly painful after a few steps to go up more than 3 or 4 stairs. If I go anywhere where I must stand up or stand in line for longer than a minute my legs weaken drastically (at the market a carriage is absolutely essential). After shopping and pitifully getting home, it takes a good day to recuperate. Been to MANY emergency rooms over the years, no diagnosis resulting. Main complaint now IS the weakness - any ideas? I have United Health Care coverage AARP Medicare Advantage Choice Plan 2 .

    By far the best explanation I have seen. i am 7 wks post Percutaneous Discectomy (July 2024), at L4/L5 S1. No improvement what so ever, had bilateral foraminal injections in L3, the L4 then L5S1 none worked, so then fact joint injection L4/L5 no response again. A June2024 MRI should central stenosis now at 2 point hence PD. Think i’m heading to open surgery where the make the hole bigger. Be glad to see the end of the sharp stabbing/burning pain down both legs.
