What is the addon that shows debuffs and major cooldowns over the target's head?

Am I blind or there is 0 Sweeping Strikes activated when the enemy is stacked? This is done for rage management? Recovery's insane btw!
Sometimes it's because of PoM bounce but I think I had a few opportunities this game to press it, not the biggest deal though
The Rend on two targets is pretty bad for PoM bounces too
