The chords are, Eb - Fm - Gm - Ab / Ab - Gm - Fm - Eb - Eb6. Play all these chords in their first inversions, starting just below middle C. Theres a few passing notes in the melody, but you'll just have to try and use your ear for those. Just cycle through those chords and you'll start to recognise the tune. The intro and outro is just Eb - Ab - Eb repeated a few times. For the left hand, just use the root notes of each chord in octaves. Listen to the original song to get a feel for the rhythm.
