Yes, that spline destruction looks very much like it has been caused by the lock nut not being locked. I seem to recall Toyota did have a problem with that at that time so it strongly suggests that's the original gearbox. Not that that's much help as that gearbox looks pretty rogered...
There is another error in the manual (depending on which manual you have, 2 very similar are available online). Following the assembly procedure at the point of tightening the big plate over the 2 bearings (x5 M8 screws) it says to go for 42nm when the correct torque is 21nm. It can be recognized looking at the exploded view (also in the manual) or checking the same procedure on the other version of the manual.

I would say the fault was the bolt by either poor fitment previous or wear. Being loose would cause the gear to chatter on the shaft therefore create the syncro to not engage fully and letting the gear pop back out also groan but at higher speed not noticeable!
Be happy you need a new trans. Rebuilding one without the proper tools and equipment would be a nightmare. By the time you buy a press, some tools to help press parts on/off the shafts and a dial indicator, you may already be at the price of a junkyard trans. I tore into a manual trans once. The clutch master cylinder failed when a novice manual transmission driver was behind the wheel on a solo drive. She didn't realize the clutch wasn't getting disengaged. "Stuff" happened and the car got stuck in first gear. In the tear down, found that a syncro was stuck to a gear. Threw everything I had at it, including fire and a 12 ton press. Broke the shaft trying to get the thing torn apart. Ended up buying a junkyard transmission. I was looking forward to the challenge of putting that thing back together too.
That was our thinking, to repair it we’d spend more on tools than the cost of a used transmission, and even then there’d be no guarantee we’d be able to fix it 😖 it was interesting to get look at gearbox internals though 😊
I'm going to guess it's an MGTF... Boxters, Elise's, exiges, vx220's are all quite expensive..... Though they did say it was a project.... Nope! It's gonna be an MGTF!
Guarantee the spline damage was due to the loose lock nut, I've had exactly the same issue happen with the output shaft to sprocket splines on my motorbike before. Without the gear retained properly it can wear sideways on the shaft and chews up the teeth like that.
