for the foreigners physical examination form, does it had to be uploaded with the negative film for Chest X-ray exams, ECG, and the examination certificates/sheets for laboratory exams? Or is the form on its own sufficient? Thanks!

No you can't. Your files will be incomplete and they really look into language proficiency levels.

When selecting 'Apply as' the options are: bachelor's degree, master's degree, doctor's degree, general scholar and senior scholar
I believe you can apply for scholarships to work in universties in China as professor etc. Maybe that's that.

For bachelor's there are no, or very few, scholarships for programs in English, mostly Chinese programs can get csc for bachelor's
