Someone whos twice your size isnt twice everything, is a magnitude factor so Cap being a super soldier gives him what is essentially to us ‘super powers’. And also on the other side he was just backhanded with no real stopping power put into it. People can actually test this themselves by backhanding say a piece of paper, no real damage gets done to it. Regular people get thrown at that G force level into stuff and survive so its not that hard to believe Cap survives it. The real test would be if Hulk had him on the ground and did a full force smash into him, now that would turn him into paste.
You can tell the exact moment Banner lost control. When he screams "this is my fight", you can just barely hear Banner's voice admist the roaring, before he then says "stay out of it" with that all too recognizable Hulk voice.
Unless that needle is made from a material that's stronger than Hulk's skin, like Adamantium, I don't think that should've penetrated through his skin. If anything, either having Hulk drink the antidote, or breathing it in a gas-like substance would make more sense.

Hulk wasn't always a mindless monster. He thought of others, and did what he could to minimize needless deaths. He definitely never really understood or cared about collateral damage though. Most of the time when he turns his anger to his allies, it's because they're disrespecting Hulk by not acknowledging him, and only addressing Bruce and giving Bruce the credit for what Hulk does.
Marvel characters don't realize how much Hulk hates being addressed as Bruce, Tony Stark kept calling him Banner in age of Ultron which made him more angry, Thor in Ragnarok too which led to there fight, This Cap too. But in when Cap called him Hulk in Avengers to smash, he was just happy, also how Black widow gets through him, and this Betty too called him Hulk too unlike Cap
