It depends. Usually yes, Lady's 1st step is outside partner. But sometimes (for example after 1-3 of Natural Turn) Lady dances in line or between Man's feet.

Is viennese waltz and waltz the same thing?

Both dances has same time signature - 3/4, three beats to one bar. Viennese Waltz is twice faster than Slow Waltz. Therefore also figures and technique are different.
