It looks like there is a support for GRAPHVIZ on the mac with MacPorts or Homebrew graphviz.org/download/ . I don't have a mac but googling a little bit I see people reporting they were able to successfully run it on Mac so it looks promising
Yes, I was able to install it using homebrew on macOS Catalina. Keep in mind that you will also need to install Java JDK (also available in homebrew) in order for PlantUML plugin to run on your machine

You can install Graphviz same as PlantUML from Visual Studio. It worked for me.
You forgot to mention that you have to install java also. Not everyone is a java monkey.
