    Have you tried adding a 4Kscore test to a 3T mpMRI scan to see if a biopsy is necessary? It seems to me that a TRUS biopsy is a shot in the dark without an mpMRI to inform it. Also, I would think that the mpMRI would show the size of the prostate, not to mention that tumors on the apex or the transition area would be seen, whereas those are often missed with random, unguided biopsies.
