I don't know if you found the answer to "why do cursed eyes show up twice?" but as someone who's still trying to get to Mother I see them way more than I'd like so if you now know the answer to that question that'd be cool. My assumption has always been that you get a secret room item spawn, let's roll the internal D-whatever to figure out the item you get, and if it's something you don't have unlocked it's a cursed eye, tada. Second Edit: I'm very drunk and still not sure if I worded it right but basically as someone who has many unlocks to yet unlock, when I find an item it's very often Cursed Eye and I have a hunch it's a default for things you haven't unlocked.
