  • In this video, we introduce Robert D. McMullen, MD who is a psychiatrist in NYC who has been performing psychopharmacology for over 30 years, the last 10 of which includes transcranial magnetic stimulation(TMS). Today, he will discuss some recent developments in research regarding TMS.
    There were recently two conferences held in Vancouver, Canada regarding TMS research. The first was a 2 day conference held by the Clinical Society of TMS, which is the U.S organization for TMS research that has been around for 10 years. The second was the three day International Brain Stimulation held every 2 years. During these conferences, new research on TMS was presented.
    Usually TMS treatments have been left excitatory on the left dorsolateral prefrontal cortex. It was then observed that if you performed inhibitory treatment on the right side it worked as well. Afterwards, many started doing bilateral treatments. Recent studies suggest that all those forms of treatment work equally as well, although for some, bilateral treatment might work better.
    Another breakthrough in TMS treatment in the last few years is theta burst stimulation. It shortens the length of treatment on both sides of the brain(40 seconds on the right and 3 minutes and 8 seconds on the left side).
    Recent findings suggest that doing 2 TMS treatments per day increases the odds of patients responding to TMS treatments. One recent study shows that people who receive 2 treatments per day for 15 days did just as well as people who did 1 treatment per day for 30 days. Using theta bursts significantly lowers the length of the treatment allowing multiple treatments to be performed in one day. Ideally, each treatment should be performed 15 minutes apart for optimal effect.
    These were just a few of the findings presented at the conferences. Although TMS research is still ongoing, the future is looking bright for this method of treatment.

    No offense to you Doc- but I hope research soon reveals that this treatment can have very bad consequences in some individuals. I had 31 bilateral treatments that resulted in me having a much worsening depression. Not every brain reacts the same to the standard treatment, and the outcomes can be very poor for certain individuals.

    Dear Dr. McMullen,
    I'm writing you from Belgium. I've come across your RUclips channel since beginning of 2019 and have been watching your videos ever since. Thanks to your great resources of information and expertise I took the decision to look for a TMS treatment in Belgium, in a well-known (academic)hospital in this country. As I don't want to come across as mistrustful to the specialist in charge of my TMS treatment by enquiring them, I'd like to ask you if it is 'normal' or better said 'meaningful' that the treatment offered here only comprises 5 minutes of TMS per day, whereas I've found many sources of TMS research indicating a minimum of 15 minutes of magnetic stimulation? I suffer from treatment resistent major depression and the area that is being stimulated so far is the left prefrontal cortex at 42(I don't know what this figure stands for but that is the gradation that is being applied so far). The aim is to have a 6 weeks treatment, no reimbursement from healthcare or private insurance with a total cost of 750 euros for a 5 minute treatment, 5 days a week. I am wondering if this duration is just way too short. I don't want to disqualify the beneficial effects of TMS by any means and my TMS therapist is a great professional. I just don't understand why the TMS session is so short, I'm curious to know what your thoughts are about it? Thank you so much for your reply. Greetings from Belgium, Catia

      They likely use theta burst stimulation that have the same effect after 3.min like 35.min of regular rTMS.

      I am a TMS Technician & I also worked front office at our clinic. This is something that your insurance may cover it requires an authorization. I was extremely skeptical prior to working there but I was able to watch this treatment change lives right before my eyes.

      It is FDA approved for depression and OCD/anxiety. So yes insurance should pay.

      @@garysimone4977 like any other treatment, the results are different for each individual. It is primarily dependent on the patient. For those who actually follow the Dr. orders and show up on time for every treatment the process moved quickly and smoothly but for those who didn't put in enough effort their results were much different. I would say do your research because everyone responds differently, from my experience I've seen it work and it an amazing transformation to witness.
