"didn't want" is a strong word, iirc Aqua didn't even know about the prank, or she is notified by Marine but chose to play along. Do remember Sara and Aqua went to a karaoke together with Norio-sensei, they're pretty much acquainted already.

It's a really antiquated way of calling someone, like Minamoto no Yoshitsune or Abe no Seimei. I don't think it's done any more, formal or not.
