Plot twist: The truck guy wanted to make this an isekai, but failed. His idea was to run him over, make him meet God, go to another world so he can get back to Earth, and then meet the girl of his dreams.
My theory: he fell into coma after he got hit by that truck, and Nasa is now dreaming in the hospital, maybe shes dreading not saving him, and she visits him in the hospital. And he hears her and hopes that all these things happen, after he wakes up. Also everytime hes able to smell her it could imply that shes visiting him. Because people in a coma can smell so the smell could be affecting the dreams, also it could be the reason why she doesnt want to be intimate with him because he never experience it, so he cant imagine it. Also she doesnt want to tell the reason why she fell in love because. He doesnt know so when she tells him its would be a sign that she told in the hospital, another thing is she has a similar personality, so that could be him imagining her as the best for himself, and the scene when there house burns down that could mean she didnt visit him in a while or in the time she promised him to visit. Another thing could be when they move to the laundry house, it implies that other people visited him and the other girl confessed to him, same is for his parents when he introduces her to them they could be asking why and he explaining in his dream now i know its supposed to be a pretty story but its just to perfect.
