Absolutely fantastic, John. A friend of mine has been diagnosed with LLD. She believes she has always had an issue and indeed she walks with a distinct "stomp".. This matter being further exacerbated by fracturing her pelvis 2 years ago. She has been advised to use orthotics but I see the delicate balance with the amount the shorter foot is raised and the potential issue of over pronation on the longer leg. Thank you!
I've been trying to figure out what is wrong with me for years. I've done countless exercises to correct pelvic tilt with no result. The way you described this showed me my actual issue.. almost without question
I have 2.5cm lld on the right side due to clubfoot. I've always been active, but lately have moved away from cycling due to it tightening hip flexors. Single leg glute bridge, mountain climbers with foot by hands and simple walking are helping. Your presentation rang true I have left side SI tightness, adductor tightness exactly as you describe. The LLD is now corrected by shoe lift but I ran long distances for many years uncorrected and my right knee now has severe OA and has 2 pins from a knee reconstruction.
That's interesting I watched the video some musical from Dr Berg and he said the last thing you should do is put a heel lift in because it drives a femur into the Elvis and it can be muscular tension causing a leg length discrepancy that should be attended to and his video was very informative

I was born with a 5cm LLD and went through a epiphysiodesis Surgery at 15 years old. After the surgery, I still have 3cm of difference (Left leg being the longest) + Scholiosis. Now at 30 years old, I am starting to experience pain on my lower back and my left QL is always tight. I use shoe lifts but I don't think is enough. I am looking at options for Leg Shortening but I really don't know what other options I have at this point.
I had similar issues. Just a lift didn’t help. But a full body Physio analysis and relevant stretching and strengthening has been working wonders. Do consider a good PT.

I still feel tightness on my left QL and on my lower back when I am sitting for too long. What sort of exercises do you do? What are some ways to fix the alignment of the hips if one hip is anterior and the other is posterior. What caused this was one of my feet have a fallen arch due to an injury a long time ago which made the hip with the fallen arch on the foot lower and the other hip go up. I can also tell the lower hip with the fallen arch has an anterior tilt and the other hip has a posterior tilt.
Some of this is a misconception. So many patients will present with foot overpronation on their SHORT leg. This is because foot pronation is not proven to be a compensation pattern where instead, it can actually be the cause of LLD.
Now a question to ask that im sure many are wondering, from an athletic perspective, what can WE do about our condition to ensure the best performance possible? Im currently 21 with a longer left leg of 1.2cm and agree with the information about the feet being different etc. I use a shoe lift but it has minimal success, could you please make a video specifically on what can be done about actually achieving the best outcomes? Theres so much information about the condition but never many optiond and or solutions to be normal
I agree about a video for a solution. I played competitive Sports on my life but now as I'm older I started having lower back pain and went and had a standing X-ray and showed I have my right femur is 1.2 CM shorter than my left. I to use insert 5 millimeter full length insert and a 5-millimeter heal lift. I'm a Competitive Cyclist so I also noticed my shorter leg and foot on the right side supinate so I have a pad in my heel leaning right foot inward. Seems to help, but like you said I love to see a great video showing corrective measures in detail for structural femur discrepancies. Especially for high-functioning athletes. What would be some good stretches exercises or supportive orthotic measures. ?
I had a similar issue, my right leg was 1" shorter due to tight glutes. I would have back pain with heavy Deadlifts or good Mornings. I got the physio to release my tight glutes and right away the discrepancy was reduced to bare minimum. I would suggest you talk to a physio who specializes in strength training to check your legs, back and hips and figure out what's causing it and figure out a plan. Check out Dr Stu McGill's videos, he has 3 exercises called Big 3, they help a lot to strengthen the spine and fix back imbalance. Hip airplanes, and Farmers Carry with weight in one hand helps a lot. I posted some videos on my Instagram for this issue, check out @coachromigill. Hope you get better soon

Sir I have the exact same problem as you described here ... I have The functional leg length difference. My right leg is long . Right foot is pronated . Right hip is high and I have pain in si joint .. scoliosis on right side...how can I cure this ?... which exercises should I do?
I have the same symptoms, except my left leg is the one that feels (I have not been told that it is yet by a doctor) longer; pronated left foot causing pain, left knee pain, left hip pain, and left SI joint pain are all happening, and have for months; I have seen so many different foot doctors and no one has been able to help me so far. Thank you for these lectures. I have a question that to date no one has been able to help me with. I have seen Chiropractors, Physio and Osteopaths. I have a LLD and a slight scoliosis. I have noticed something else. When I stand, spread my legs beyond should width, the left leg comes off the hip on an angle. The right leg does not or does not as much. If I then bend from the waist the right hip is obviously high when viewed from behind. It is difficult to explain. I have been doing yoga exercises to improve hip range of motion. I am a very active 57 year old and while I am able to run and cycle with no real pain it is obvious that something is wrong. Thank you in advance.

Hi John, Can you have both a Trandelenburg and a Compensatory Trandelenburg one on either side? if possible I have had this for a long while and I ended up standing primarily on my left leg and can barely support my torso as my Gmax does not want to engage on the right. . I use my right QL to hold up my hip. Is this possible. If you could reply it would be most helpful when I see a Physio
John Gibbons agreed , although they refuse to give me one. The podiatrist used a measure tape & said it was for sure structural so no X-ray needed. I’ll have to come & visit you someday !
Is there any correlation between a longer leg and size of chin bone? It's weird, but the right side of my chin is about 1/8 in to 1/4 in longer than the left side. This developed in my teenage yests. I have scoliosis, a thorasic C curve as far as I can tell, with sciatic pain around my left L5 and left side middle of my shoulder blades. I haven't been measured, but I suspect my right leg may be longer.

Most of the text has talked about true lld, which causes hip hike on longer leg and pronation of feet on the same side. I have been waiting for correction of over 1½" discrepancy of my right femur following a surgery that wasn't really done right. During the time since I've been slowly losing my pelvis and other mobility. I'm 28 so I'm sure I can heal but my pelvis seems locked. I had a spinal fusion on l5 to s1 for a pars defect injury. Slipped disc in high school. It was the strongest part of my back after pt following surgery. Now it's almost like it's trying to hold to protect itself. I understand nothing will stop this until I finally get my leg lengthening procedure hopefully soon. It seems like pronation has gotten worse and my left leg is almost carrying the pelvis and my right is sort of just inactive. Whether or not you can help I'd really like an experts opinion on some things I can do to get my life back. I can't activate glutes or kick heels. Arches gone. If I should email please let me know but I'd love to hear anything you might have to say about that I'm still watching and thank you for posting this
Hey there I have a tilted pelvis like in the photos however it never used to be like this, I'm 30 noiw and id say in last few years it's gotten worse also bcs I've been inactive since age 17 pretty much gaming etc sitting in bad posture etc anyway I really feel like my right leg is shorter but also the glutes and muscles in general on my right side are smaller and when I tense my weak right glute everything evens out into perfect posture I also bad apt and I don't know what the cause is (weak core bcs when I flex my belly again everything lines up and goes into place but the pressure holding that position hurts I've bee to so many ppl gotten adjusted etc but no one gives me proper answers it's near impossible to find someone who is really willing to help and I live in London I also have a knot or something that's cropped up in last 2 years also in my right side oblique area at the back it's like constant locked but sometimes I tilt my body to other side and feel clicks and release all the way up my side I just don't know what's going on anymore, I can't get on with my life at all bcs this is on my mind 24/7 every step, I just want to feel normal and be able to train fully without being deformed like when I workout hard one shoulder is bigger bcs my hips or something must dip during pushup as the weak side is rotated forward knee pointing in and foot flat unless I force it which just feels tight... I'm at a loss and gotten to a stage where I just think of suicide nd these thoughts never came to. My head before as I'm usually a happy guy but with my long hours job and the constant pain with no answers from any 'professionals' I just feel like giving up, everyday is a pain nd I'm only 30

I broke myv ankle. i did not had surgery and now i sm almost fully recovered there but due to about 2 mm leg dicrapancy. i feel it prevent me from learning walking. my question can that feeling that i never had before breakingg my ankle can be caused by short muscles ? After all had quite in low to no physical activity for 3 months.
If it is only 2mm then i wouldn't worry about it and i doubt the fractured ankle has caused the leg length discrepancy and was probably already present, regards JG
Dr. Gibbons, at 7:09 you've said that the innominate will rotate posteriorly on the side of the true long leg in order to compensate. But here at 11:36 you are speaking about a true long leg and that the innominate tends to rotate anteriorly. Can you please help me to understand this? I understand that a anteversion of the innominate results in a longer leg, but here we are speaking about true long legs and its compensations. Please give me a hand with this. Thank you.
What should I do to fix this at home? I rotated my hips during a splits stretch. I felt something gliding and a sharp pain. I can't do the splits since and one leg is now "shorter" and the opposite knee is overcompensating and overpronate. I am still looking for the exercise that will readjust my pelvis and sacrum.

I've seen many chiropractors who do absolutely nothing and many osteopaths who only care about your problem if it's something they can operate. US medical system is not designed to dig deep into the root of the problem. It's designed to treat the symptoms. Your thoroughness is unseen here.
Dr thanks so much. I had an X-ray done and have 12 millimeter shorter right femur. From your video it looks like my longer left foot will supinate correct? So I can a shim my left foot on the lateral side correct? I'm a huge cyclist also do you have any advice on rotating the saddle either side because of the pelvic tilt?
Hi John great explanation even I could understand lol been to nhs specialist she said I have long leg descrepancy suffers pain for 30 years she made inner soles no more pack lower back pain and shoulders 100 percent better but had mettarsalgia now almost gone but still have hip pain in nights and ankle swell painful any ideas to help ankles and hip pain everyday Learnt many stretches of years and suggestion to help hip ankle pain on both sides many thanks Dave
It would depend if you limp from pain or from weakness. Try getting your glutes needles if it’s from hip pain. Bridges and clamshells are great beginner exercises for weakness

My right hip is lower then my left. And when I stand on my right foot I’m about 3/4 inch shorter then on my left. What does that say?

I have had a stroke at the age of 10 and now I’m 62.I have a drop foot right side with about One inch leg difference and pelvic telt .

when innomate rotate anterior ,ischium and acetabulum moves back , it should make leg shorter !!!! correct me if i am wrong. Thanks
Anterior rotation makes the leg appear longer - especially in supine. regards jG

I have a shorter left leg , opinions or wether it’s structural or functional are disputed . But I have bad issues with my left foot & right knee 😭
You will have some issues if there is a leg length issue, best way to measure is through an x-ray, regards JG

I had a compound fracture as a young boy in my left fermer I have a exray and their is a half inch difference in length what would you recommend
Heel lift is the easiest option and worth a try initially to see how the symptoms are / change, regards jG

Hi I have to accident 11 month before femur bone loos also after bone graft surgery now status fracture leg 1, inches shorter still never remove screws total 14, number of screws knee bend 90, degree after removing all the screws my leg can come normal pls let me know 🙏

I broke myv ankle. i did not had surgery and now i sm almost fully recovered there but due to about 2 mm leg dicrapancy. i feel it prevent me from learning walking. my question can that feeling that i never had before breakingg my ankle can be caused by short muscles ? After all had quite in low to no physical activity for 3 months.
If it is only 2mm then i wouldn't worry about it and i doubt the fractured ankle has caused the leg length discrepancy and was probably already present, regards JG

Dr. Gibbons, at 7:09 you've said that the innominate will rotate posteriorly on the side of the true long leg in order to compensate. But here at 11:36 you are speaking about a true long leg and that the innominate tends to rotate anteriorly. Can you please help me to understand this? I understand that a anteversion of the innominate results in a longer leg, but here we are speaking about true long legs and its compensations. Please give me a hand with this. Thank you.

What should I do to fix this at home?
I rotated my hips during a splits stretch. I felt something gliding and a sharp pain. I can't do the splits since and one leg is now "shorter" and the opposite knee is overcompensating and overpronate. I am still looking for the exercise that will readjust my pelvis and sacrum.

Great explanation. So does most people present with a short or long femur? The femur seems to be the start of these cascade of joint problems.
If a true leg length then yes the femur can be the problem regards JG

@@JohnGibbons I think I might have to go outside of US to find a knowledgable person. I've seen many chiropractors who do absolutely nothing and many osteopaths who only care about your problem if it's something they can operate. US medical system is not designed to dig deep into the root of the problem. It's designed to treat the symptoms. Your thoroughness is unseen here.

Dr thanks so much. I had an X-ray done and have 12 millimeter shorter right femur. From your video it looks like my longer left foot will supinate correct? So I can a shim my left foot on the lateral side correct? I'm a huge cyclist also do you have any advice on rotating the saddle either side because of the pelvic tilt?
My friend has right long leg due to weak gluetal muscles. How can it be treated?

Hi John great explanation even I could understand lol been to nhs specialist she said I have long leg descrepancy suffers pain for 30 years she made inner soles no more pack lower back pain and shoulders 100 percent better but had mettarsalgia now almost gone but still have hip pain in nights and ankle swell painful any ideas to help ankles and hip pain everyday Learnt many stretches of years and suggestion to help hip ankle pain on both sides many thanks Dave

Any recommended exercises for those who limp due to a femur surgery
It would depend if you limp from pain or from weakness. Try getting your glutes needles if it’s from hip pain. Bridges and clamshells are great beginner exercises for weakness
