For those having problems with the Ctl + Alt +drag, make sure you're using the correct tool: If you have the move tool selected, you'll duplicate the layer. If you have the quick select tool, brush tool, eraser tool (anything that uses the brush engine) Ctrl + Alt + drag will change the size and hardness of your brush. Happy Photoshopping! :D
This tool is really hit or miss, and you'll want to make sure you're using it with an image that has well defined differences between your subject and the background.
Yeah, that worked well because the bottom part of the cat intersected with an almost pure black blanket. It doesn't work that well when your background is more complicated.
This tool does not really do the best job in the world, it's adequate for some work flows, but I prefer using channel masks and then modifying it with layer masks.

It works with just Alt + (hold and drag) Right mouse button

before click to Refine Edge go to the menu Selection > press MAIUS and click on Refine Edge and see what's happend. Cheers
