I had a problem with this trophy on PS. When you run, you have to turn back at the right moment for the enemy to appear. you also have to complete the level with four types of weapons! The number of hearts doesn't matter in my opinion.
Thanks for this additional tip. I played on Xbox and this method worked, but for some reason seems to work slightly different on PS.

It seems for the first enemy to spawn you ahve to be super quick.
It would sometimes spawn when I was slow if I had more than 6 lives, so I went with lives than speed as my thought for how to make the enemy spawn. But I don't know the exact method. Only thing that is for sure is that you have to get all 5 of them in one run to get this achievement to unlock.

Interesting. In my plays on Xbox, he always appeared but PS version might be different. Do you still get the trophy without killing that enemy?

I will probably do a full guide on this one. Many people seemed to have trouble with this particular achievement so I put up a video first.
