The running theory of that song is that they're in two parallel worlds where the one of the boy is after a nuclear war and the two of them meet underground and the boy vomiting blood in the end is sign of radiation poisoning. There's also an answer song for this called This World to You (Kimi ni Sekai)
Another theory is that both worlds are representations of the boy's psyche, 1 being how he sees reality after having been lethally exposed to radiation and the other being an escapist dream world for him to briefly forget his troubles. Both worlds start to crumble at the end because the boy apparently is told of just how severe his situation is and how little time he has, hence why he blows up while the girl slowly disintegrates with him.
Coz the radiation of the nuclear is freakin dangerous because even if you survive you will get the radiation that can give u a cancer, if you look closely in the video there is a lot of planes, it is representing the war between countries, and when you see at the end thr boy spit a blood which is his time is near and its about to end.
