    Good video Dr shannon! in fact i've just published a similar video as i've started with the mix and match approach on select cases with very good results! the trick is to have in account the natural aberrarions of the cornea and pupil size! Good to see such a respected surgeon also reporting feedback with these new iol technologies

    I did one eye for distance and one for intermediary and it works beautifully. I was told by my doctor that it works really well for about half of patients

    36 year old here with Posterior Subcapsular Cataracts and about to go through this, I’m glad to hear it’s possible and helpful! Though, I’ve only had my right eye done with the J&J Synergy Lens, its distance vision is terrible for me, mid range is ok, near vision is good, but a lot of dysphotopsia. I’m looking to get my left eye done with the Vivity to help restore my distance vision and fix the dysphotopsia. This video couldn’t have been better timed for my use case, thanks Dr. Wong! Wish I was in TX to choose you as my surgeon!

      Thank you for you feedback. If I am not mistaken both Panoptix and Vivity are now three years old so newer more effective lenses should be hitting the market in 2024. I would advise everyone to hold off on those older lenses. The newer lenses will be truly revolutionary and will offer a superior range of vision compared to Vivity and Panoptix.

    I had restor multifocal toric iol in my right eye and panoptix multifocal toric iol in my left eye, they work together very well, almost excellent

      Didn’t you noticed any Halo effect after cataract surgery ? If you noticed, how much time it took for halos to go away ?

      @@manijha6774 in my right eye, after the excision of the membranes in the anterior chamber, I no longer have night halos around light sources, and in my left eye, four years after the operation, I still have night halos

    The more videos i watch the more i think vivity is the all around best option. None are perfect of course but it seems to be the best compromise

    Can you address this? 66 year-old white male born with amblyopia in the left eye. This resulted in monocular vision while maturing and adapting. This person played 3 sports growing up and is still proficient with old-school firearms with respect to using open sights rather than scopes or other optics. Recently, the guy had cataract surgery on the right eye only. With a standard intraocular lens, midrange and far vision much improved, but close up still requires reading glasses.
    My question is, how does an old guy like that respond to having this done on the left eye with the addition of a multi-focal lens to help compensate enough to roughly match the newly improved right eye?
    Seems like the old guy would have to relearn activities performed for over 60 years.
    You suddenly have pretty good binocular vision. What about driving? Catching a ball? Estimating distance?
    I don't know sounds like a real challenge to me.

    I wonder what percentage of patients don’t tolerate having one eye for distance and the other eye for close and intermediary?

    This doc is something else. My doc told me cataract procedure is irreversible.

      I think an exchange is possible and less risky the closer it is done to the date of the original surgery. I brought up the irreversibility issue during my consult and the doctor said it could be exchanged if you didn't like it. But many of the problems I see people complain about don't happen until months later, or they were told to let the eye heal and settle down first and it never did.

    hello Dr.Wong.Hello Doctor! My mother aged 64, with a high myopia of around -8.75 and -9.75 had her cataract surgery with the combination Panoptix toric in left and synergy toric in the right. While synergy went in first, her post surgery vision and the experience was Fantastic almost immediately I.e almost all distances had a high clarity. The Panoptix was done next. However there was a slight preoperative discomfort and she had an edema post op and was def not satisfied with the vision almost immediately but it got better the second day including the irritation. The distance and overall vision now is better with panoptix. However when focusing with both eyes she sees a double vision I.e jumbled letters, lines etc specifically for intermediate vision.On a review with docs they said it will take time for neuroptical adjustment and will be better with time ! Can you please advise if this will be and in your experience how long might it take. Shall highly appreciate your response

    Thanks to Dr. Wong for your series very informative videos that is great help for us the cataract patient.
    I just received a vivity lens replacement 5 days ago, the results are not satisfactory. My far vision only 20/30, worse than before surgery, when I have glasses is 20/20.
    Is there are someway to make after surgery adjustment to improve my for vision on vivity lens?
    Can I put in a PanOptix lens in others eyes to help me for near sight reading?

    After watching you video, I thought that I might select prime lens to see well far distance such as driving or watching TV without a pair of glasses. But the eye doctor says it will be inconvenient for me to read the dashboard when I am driving. So he suggests that I just need to replace the cataract lenses implant the artificial lenses and continue to wear bifocal glasses which I currently use now. Confused 😂

    I had cataract surgery in my left eye several years ago. I had astigmatism so a toric lens was used. It left me quite nearsighted in my left eye. I have a cataract developing in my right eye so trying to decide what lens to use for that eye. My left eye is my dominant eye. Everything I have read suggests it is best to set your dominant eye for distance and non-dominant eye for near vision. Considering PRK for left eye to switch to distance. How important do you think it is to have good distance in dominant eye/near for non-dominant, versus the opposite which I would have if I don't have PRK in left eye. (My ophthalmologist is recommending LAL for right eye assuming I leave left eye as is to give me good distance vision without glasses.) Thanks

    Hi ,can you put only PanOptix lens multifokus on the dominant eye only (due to damage from tiny hot object later turned to cataract they told )and left eye is ok i use glasses for near since happened .they say we canNOT put PanOptix multifokus on the dominant eye,thanks

    Did the patient adjust to the negative dysphotopsias in the right eye or did you need to lens exchange the Panoptix lens in the right eye with reverse optic capture?

      I am the patient. The dysphotopsia is still there on both eyes, but I’ve learned to live with it.

    Thank your for sharing Dr. Shannon, great video! Do you change the IOL Power when planning a Reverse optic capture or just go with the in the bag power calculation?

      An IOL in ROC will need to be a lower power than an IOL in the bag.

    I think it is not worth to use anyone of those lenses. Monofocal provides the best contrast and much better...

      I am the patient in the video. I was tired of my progressive lenses and didn’t want to wear readers, so it was worth the cost and hassle for the surgery.

      @@mlea5327 I just had the right cataract removed and a clareon Vivity tonic lense installed on June 17, 2024. Only been a week but my distance and intermediate vision is excellent. Good close up near vision is good 40 cm and out. Can read small print 3 mm in size at 40 cm. I did notice that in bright light out side my near vision is 20 cm and clear. So light makes a big difference. Totally happy with the outcome. No halo’s or floaters. Some flashes for 2 days then none. Looking forward to getting the left eye done on July 29, 2024. For fine close up work will need cheater glasses. I’m OK with that. 90% of the time won’t need glasses. This is the best vision I’ve had in 20 years. My age is 65

    Do u calculate IOL power acc to the reverse optic capture or the same as IN BAG CALCULATION

      Yes. An IOL in ROC will need to be a lower power than an IOL in the bag.

    A little off-topic, but, does it work to mix one eye with a natural lens and the other with an IOL ( monofocal or multifocal)? I have one eye with a cataract which definitely impairs vision. The other has the beginning of a cataract but has been stable in prescription for the last 10 years. Post surgery, I don't mind wearing glasses.

      It does, I have panoptix in my non dominant , and my natural lense in my dominant. Works beautifully, I have amazing vision from 35 cm to far. Shannon also has one eye with Symfiny lense and the other eye the nstural lense.

    Very informative. This addresses using a different kind of multifocal lens in each eye but is it possible to have a multifocal lens in one eye and a monofocal lens in the other?
