    distance of elementary part from the center of mass you have taken as y because you considered the elementary part as a rod. If you take it as a cube, then distance from center of mass instead of being just y, becomes r = SqRoot(y^2 + x^2), MOI of elementary becomes dIzz = dm * r^2 = dm * (x^2 + y^2); dm = (M/bd)dxdy; After integration Izz = M/12(b^2 + d^2) and similarly Ixx = M/12(d^2 + t^2) not just M*(d^2) / 12. Your solution is a wrong for a 3d plate. For a 2D plate its right but since you have shown a 3D plate with thickness t, then M/12(d^2 + t^2) seems right to me. I dont know.

    This is wrong due to the fact that you treated the rectangular plate as a rod, a plate has no thickness
