- When it comes to comparing transPRK to LASIK today, things have certainly changed. Modern bladeless and touch-free transPRK is not comparable to the original PRK method from the 1990s. Also, a transPRK is safer than LASIK surgery.
I just got PRK 5 days ago and I gotta be honest the recovery on the 2 and 3rd day SUCKS my eyes were burning so bad, but then on the 5th day it was like a switch and everything stopped burning and my vision started to instantly improve, such a crazy process
As a martial artist and footballer, I had to undergoe transprk 6 days ago in order to continue playing the sports I love.
Indeed it's much safer than all Lasik-Smile surgeries but it takes longer to recover and was very painful during the first 48 hours after surgery, other than that, I have had good vision and I even ordered a fancy monochromatic sunglasses 🕶️🕶️

I got my FEMTO LASIK done 3 days back but I wish I had watched your video before it as you mention TRANS PRK is more stable long term, and your logic does make sense. I was turned off by it due to my nature of work and the doctor mentioning that PRK takes about 1 month to show full results while LASIK shows results instantly and full recovery in 1-2 weeks.
I would not worry too much. Femto-LASIK also is very safe. When I say that transPRK per se is safer than Femto-LASIK, then we speak of two methods that both are safe, but one is even safer than the other. Because in medicine, it is all about making good therapies even better.

I am a bit relaxed now with what you've said, however, could you also clarify if the effects of Femto LASIK wear off faster than PRK and will my flap dislodge anytime in the future considering I will be getting into combat sports like MMA or Boxing?

Thank you so much for this explainer! Having the development of the field and techniques laid out like this helps make sense of the options available now. I learned my cornea isn’t thick enough for Lasik and have access to transPRK (with Streamlight).
I still wonder about hazing during the recovery process and beyond. I was told the first months are critical to shield the eyes from UV light, but also that one needs to be careful for 1-2 years afterwards, and that always wearing sunglasses on sunny days should be best practice for the rest of one’s life.
Am I simply overreacting and overthinking how careful one must be, and / or how sensitive are the eyes to hazing after the first months of the surgery? Apologies if you already have a video answering this question, thank you already for this one.

@@lililiz7869 the procedure was very quick. As for me since I have high grade, my right eye took about less than a minute whereas my left eye took 1minute more or less. After 5 months now, no regrets for the procedure. It's okay to feel nervous but always remember it is always a step towards a clearer sight for a long time. Bye glasses or contacts soon for you.

I'm still considering ICL due to its reversibility as opposed to laser options due to the risk of severe chronic dry eye. My mother and uncle both had LASIK, and still have severe dry eye 10 and 14 years later, so I am very afraid of laser eye surgery.
Hello! Indeed, LASIK may lead to dry eye, because the nerve endings of the corneal nerves are compromised. This may also occur in transPRK, but much less.
@@ELZA-Institute Thank you for the reply sir. I just had standard modern PRK (brush epithilial ablation) on May 3rd. So far my eyes are still stabilizing other than night time dry eye is my only issue.

@@icdlight9284 my eyes are slightly noticeably dryer and more sensitive to light. That's to be expected and recovery is stated to take a full year. It's not bad at all and God blessed me with a miraculous recovery; even yhe Doctors thought my surgery date was incorrect in the file becasue I was so ahead of schedule in recovery.
If you do PRK be absolutely sure to protect your eyes from UV for at least a full year.
Absolutely would do it if I had it to do again. The hardest part was my eyes are 10x more sensitive to touch and I am immune to the effects of tranquilizers so no calming or sedation for the surgery and it was nearly impossible to hold still and not blink.

@@torontogamerz794 Their is risk in all refractive surgery, however TransPRK is the safest method, their are alternatives you could also consider such Visian ICL surgery (if you have a prescription that is higher than -8).

@@torontogamerz794 Ok this is a minor prescription that can easily be dealt with, I would suggest you avoid Lasik and pursue surface treatment such as TransPRK or LASEK (They are essentially the same procedure but TransPRK gives quicker and arguably safer results). Visian ICL is not performed on eyes that are below -3.

Is there currently a therapeutic surgery for Post Lasik ectasia? I developed corneal ectasia only in my left eye 5 years ago. I had Crosslinking in 2016 and it has been stable since then.

I agree that the advantages of trans PRK (as known as streamlight PRK that I'm booked in for this week) outweighs any negatives.
However, I believe that trans PRK destroys Bowman's layer in a way LASIK does not. Whilst I appreciate that there is no medical evidence that supports the role of this layer, is this something to be overly concerned with?
Is there anything I can do to ensure the surgeon enlisted for the procedure has the required skill and success rates?

LASEK has been more or less abandoned in the past years because the epithelium is not really viable after the alcohol treatment.

I have a 2 part question. I am 2 weeks away from getting laser surgery. I was not quite sure if I would prefer LASIK or PRK ...but after this video I think I will go with PRK. My question is, being that I apparently have thick cornea tissue (in the 500s), is PRK still a better option? And if so, is transPRK an even better option than PRK? I am not quite sure of which my doctor uses.

Hi. Please note that PRK and transPRK are quite different. A PRK procedure can also be performed on a 15 year-old-technology with older ablation algorithms and poorer outcome. Similar in LASIK: a modern Femto-LASIK shares the name with LASIK with a blade system, but is quite a different procedure.
@@ELZA-Institute Dr. Farhad, this patient asks some good questions. In cases where a healthy thickness of cornea is identified and the procedure will not render a post operative coronal thickness of less than 300 microns, is your logic of Trans PRK instead of LASIK still valid?
Also, is Trans PRK greatly more advisable than PRK (I think you did answer this in your reply in fairness)?
PRK disrupts corneal structure affecting epithelium, Bowman's membrane (never grows back), and anterior stroma (never grows back). Epithelium healing is a complex process. With corneal regeneration many inflammation and complications can occur through the life but with flap dislodge its extremely rare less complication. ( if occurred also its again its very very rare).
If you touch or poke with finger or sky dive still flap wont move after 6 months. And by the time you damage flap your eyes structure will be deeply damaged by all means. Whether u had lasik flap or not, in accidents your eyes will be damaged. In healing process if small accident occurred also flap portion can be easily placed on eye with eye drops
The other problem associated with the PRK, is the increase in a corneal haze with the treatment of higher refractive errors. It is because the epithelium cells can never be removed completely, by either alcohol or laser and these tend to grow more haphazardly.
There is also an increased risk of infectious keratitis or corneal ulcer with surface ablation compared to LASIK due to the longer epithelial healing period since the protective epithelium has been removed.
Remember, everytime you get a corneal ulcer, you are going to get another scar. And the next scar might be in the central cornea, if you are unlucky enough, and that would permanently affect vision. The eye is a pretty fragile thing, and some things, once they happen, may not be reversible. The most important thing is to prevent such things from ever happening again.
You are right, It all is a question of assessing the individual risk.

Hi Doctor. I'm convinced that trans PRK is the best option and I'm undergoing the surgery (smartsurface trans prk) in Singapore in two weeks. Due to the high correction required, (-500 + 200 astigmatism), my doctor recommended corneal cross linking. Is there anything I should take note with regards to recovery? For example, I've read that consuming flaxseeds and more omega 6 can help with recovery though it might be a placebo effect. Thanks for reading :)

right eye: spherical: -5.50, cylindrical: -0.50, axis: 15°, acuity: 6/6P
Left eye: spherical: -5.75, cylindrical: -2.00 axis: 5° , acuity: 6/6P
Had my pre LASIK test. The surgeon said that the cornea is thin, about 508 um so 6/6 couldn't be possible with LASIK, I may need to wear -1 glasses after LASIK. He suggested me ICL which is very very expensive here.
My question is, Can PRK correct my vision around 6/6 with this thin cornea? Am I a good candidate for PRK? Can PRK correct my astigmatism?
If you can afford to take a week or two off work, PRK is a no-brainer. Yes, it takes longer to recover. But it keeps your eye much, much healthier.
But unlike LASIK, you lose Bowman's Layer forever as it'll never regenerate.
@@da_great_mogul That doesn't really mean anything. It's a layer of cells that perform no critical function. Hell, their most important function is to be removed to have PRK done.

@@da_great_mogul Nope, but I've read all the research papers and talked to my surgeon before choosing PRK. Bowman's Layer does not regenerate, but the other cell layers do, and they regenerate over where Bowman's membrane was. Effectively they replace it, and so far, nobody has any idea what Bowman's membrane does. Altering your eyes has repercussions either way--LASIK creates a permanent flap and cuts deeper into the tissue, resulting in the risk of less eye tensile strength. Both procedures are overwhelmingly safe and effective though.
@@L11ghtman I'm not sure about the permanent flap created by lasik to be honest. It's a tissue so the slit will surely heal.

Hi, yes, absolutely. But ICLs come with other risks including increased intraocular pressure or early cataract formation. Please consult with your treating surgeon.

Dear Doctor, I have done Lasik in my early 20s and now in my early 50s. I couldn't do the Femto-Lasik due to my thin cornea.
Due to aging, my Myopic power has increased since my last Lasik and now with Hyperopia, and possibly of Cataract in the near future, would you suggest it would be good to go for TransPRK ?

Do you feel the streamlight system is a safe and effective alternative to the Schwind Amaris system? Or would you try to find a clinic using the latest Schwind Amaris system? I am only aware of one clinic in my current country (Colombia) that has the system you are using.
I have no experience with that Alcon system and method because it is new to the market.
Michael, the Schwind Amaris is the machine used to correct your vision.
You can pair trans (streamlight) PRK with a customised treatment laser all with the Schwind Amaris machine.
Likewise, the same can be said for the Alcon EX500 machine also. These are machines, not techniques. Think of BMW or Mercedes-Benz driving you to the next city. They're both making the same journey but it's via two differing motor vehicle manufacturers.
Do you have a maximum negative diopter you treat with tprk? Edit: given a corneal thickness you would deem thick enough.
Thanks for the information doctor! I had iLASIK 7 years ago, but my right eye got a bit worse in the past few years. Now I have about 0.75 of astigmatism. My doctor recommended TransPRK over lasik, since my flap is probably too healed to lift it up again. Do you think I have a higher chance of side-effects or failure in the TransPRK procedure since I had ilasik in the past?
It is typical to perform a second procedure as transPRK, this is what I do in most cases of older LASIK, too.
How did it go?
@@evafraga1706 It went really well. The recovery from the TransPRK was much slower than with lasik, but I recovered well anyway with 20/20 vision.

Hi Doctor, I'm currently considering Smile and transPRK but still don't know which to choose. My eyesight is -5.00 with 1.25 and 1.50 astigmatism, and the cornea thickness is around 530. I prefer transPRK but I'm also concerned about getting haze, also I've got acnes before and may I know will it increase the possibility of getting haze post operation, please? I'm thinking about Smile as well but I'm also worried it will leave my cornea too thin and would cause keratoconus, especially in the long term. May I ask which one would you recommend, please? :))

Hi Doctor, I got my trans PRK done 5 days back ( goes by the name T.E.S.T at the clinic), but they used a SOLID STATE CUSTOMVIS PULZAR Z1 machine and not SCHWIND. They said it had more advantages in terms of temperature and no excimer. Is it true ? Or Schwind is the best machine in current times? Which one do you recommend?

Hello: for myopia and astigmatism as well as for slight hyperopia (up to +2 diopters), I would choose transPRK. For higher hyperopia with astigmatism, I would choose Femto-LASIK.

Yes, absolutely, but in very deep ablations, the time to healing is longer. Up to -8D, the precision is very high, then it is lower without an increased risk profile.

Hi. Yes, petfectly normal after 2 weeks. Make sure to see your surgeon in the next 2 weeks.

I have -4.5 and -4.75 with cornea thickness of 485mm. My surgeon recommended me Relex over Lasik to ensure cornea stability because of the thinness of my cornea. Unfortunately she does not offer TransPrk. Even though I paid for all the initial exams I now look for a surgeon who offers transPrk after watching your videos.
What is the min thickness of the cornea transPrk is performed?

Hi. transPRK is touchless so there is no interference from a blade. In moderately myopic eyes, I would prefer transPRK because whenever there is an additional step (pocket and lenticule creation in SMILE), there is room for additional complications.

Theoretically yes, but the number of ectasias reported after PRK is extremely low. To my knowledge have ever been reported a few years after PRK or SMILE during pregnancy. If transPRK is not available, then standard PRK is not really a good option with these values. The rest depends a lot on your individual topographical and refractive data.

Hi. SBK is a good option whenever a transepithelial excimer laser is not available. A modern transPRK laser makes the SBK obsolete.

I know that a number of surgeons use transPRK in India, i.e. those who use the Schwind Amaris platform.
Thanks! The laser in question is the AMARIS from Schwind and to my knowledge there are a number of clinics in India using that laser.
In case someone has acne that affects the eye making it look red(small veins visible around the cornea) and is advised not to wear contacts as they can lead to an infection, as long as they are totally viable for the upcoming procedure, would trans prk be a good option over the femto lasik procedure? I'm making this question as post trans prk you got to have contacts for a couple of days constantly.
This is a special bandage contact lens and it cannot be compared to the usual contact lens wearing.

I would always prefer a surface treatment via transPRK, if the examination is in fav our of it.
Hello doctor. I had prk surgery the older version in 2015 my cornea thickness was 530mm. I was 22 yrs when I did prk. I had cyl -3 both eyes. Got pregnant at 25 yrs n my vision started becoming blurr, now at 28 I have 0.75 cyl power, -1.75 sph power I'm wearing spectacles again, now my corneal thickness is 457 mm . Feel like the surgery went for a toss. Kindly suggest what should I do?
You were far too young to do anything on eyes that were clearly still not stable. Would you cut a newly laid grass lawn? You might be OK for trans PRK given you only need a very small amount of tissue removing from your cornea. However, please wait until you're 32 to 35 for that.

I am considering TransPRK, I am -8.5 in both eyes and age 43, as of yet I am not experiencing Presbyopia so should I avoid any monovision treatment and is it better to leave my soft contact lenses out 2 months prior to consultation or two weeks , would it make any difference to the final outcome of surgery.Regards Dr Hafezi

Hi. If the specifics of your cornea allow for it, I would choose transPRK over LASIK. We can control haze up to 12 D of myopia.

In such a case, and if all other factors are in favour of it (symmetric topography of anterior and posterior corneal surface, age, etc), then a transPRK makes much more sense than LASIK because one spares a lot of precious tissue with it.

1. in PRK, there is no risk of ingrowth, because there is no flap under which cells can grow
2. I use transPRK since 2010 and my video describes the advantage of transPRK over LASIK.

Hi. I'm considering to have PRK in only one eye, to correct astigmatism of -2.50 , with no spherical correction. I realize PRK is less invasive than LASIK and doesn't have the potential risk with the flap that comes with LASIK. My question is, once healed, is the epithelium/eye as strong after PRK as it is naturally before surgery - in terms of possible injury, wind, sun etc? I mean after fully healed, 6 month post-op onward. Also, while I realize PRK is less apt to cause dry eye than LASIK, would someone having PRK only on one eye generally experience more dry eye in that eye than in the eye not operated on? Again, I mean after the eye is fully healed, after 6 month. Thank you.
I had PRK done and both eyes healed in different way. My left eye was very dry while my right eye i didn't experienced any dryness at all. Was 20/15 after 3 weeks with my right eye and 20/30 with my left. After time (3 months) my left pair out with my right eye at 20/15. So I guess if you're having the procedure in only 1 eye it would be hard to compare how fast or slow your eyes are healing. Dryness went away after 8 weeks. My prescription was -6.00 right and -5.50 left. Just make sure you get the procedure done with an eye doctor that doesn't work in a lasik institution those places are shaddy!

If your cornea allows for it regarding topography, I would do a transPRK, because your corneal biomechanics will be much better.

Hi. If you did not notice floaters before the surgery, then they will typically disappear within a few months after surgery. Please have them checked regularly,

Sir ...I had prk in 2019 pre prk I had simple myopia and my corneal thickness is 499 microns in right and 489 in left ..after surgery I achieved 20/20 vision but I saw floaters little bit ...I have an experience with that floaters since my childhood but post prk one year later it was very noticeable and dryness is also increased when I woke up....upto one year my vision is perfect and no dry eyes and issues but after completion of one year I facing the so many minor complications my night vision is also very disturbing seeing stars and glare .....why all this was happening to me....please help Dr.

@@ravenmilian63 It can. It always is a question of the dosage, as Paracelsus said. So when a surgeon exaggerates by removing too much tissue, then even a PRK may develop ectasia. But the relative risk for ectasia is far lower in PRK when compared to LASIK or SMILE. And in surgery, it is always about reducing the risk.
@Salty Kraken Bowman's membrane does not contribute to corneal biomechanics. We have done the research and the manuscript has recently been submitted. For all other issues, I strongly suggest to talk to your treating surgeon. You must be comfortable and he/she should answer all open questions. Regarding epithelial removal: this is a standard in Europe since 2008, it is called transPRK.

Would you saw its safe for someone with custom intralase lasik to participate in water sports like water tubing and water skiing? It seems like the flap is pretty weak after surgery. I am not confident that a lasik flap can withstand a hard impact on the water like what is common with these sports. My surgeon says otherwise.

Hi. Let me answer in a general way. The flap has good adhesion to the underlying wound bed, but biomechanically, it takes very long to contribute to biomechanics. And I had a patient of mine 15 years ago who unfortunately received a leaf of a plant into her eye 3 years after Femto-LASIK. The leaf went into the interface and sheared off the lamella. So mechanical stress can, under certain circumstances, become a real danger to the integrity of the flap. This is why, i.e. in Switzerland, professional pilots cannot have LASIK surgery. PRK (and transPRK) are ok to perform. So if you have a profession or are performing a sport that runs the risk of mechanical impacts, a surface ablation, if applicable, is potentially safer.
Oh okay. So as long as a solid object doesnt get under, or sheer the flap it shouldnt be a problem for the most part. Also, i can only assume that a patient you may have seen 15 years ago probably received bladed lasik, instead of the more modern all laser lasik. Im kindof amazed that it only took a leaf to dislodge her cornea regardless of what procedure she received. Would you say that the technological advancements from bladed lasik to intralase lasik help keep the flap down when it is undergoing abuse from blunt impacts?
@@stupidmilkshake4887 I was looking for the right term: it was a leaf from a yucca, so definitely a very robust leaf. Whenever there is a chance of a blunt impact, I would not opt for LASIK.
That is good information. It seems i made a terrible mistake then as my surgeon assured me of its safety. I am into contact sports and such. He never mentioned PRK to me despite him knowing I am a younger man that has a rougher lifestyle. That is very upsetting to hear how fragile my eyes are now. Thank you for being honest. transPRK has been in clinical use in Europe and most parts of the world since 2005. For now, to my knowledge,it is not available in the US yet.

when the laser work, then our eyes moved, what will happened? I ever had this surgery. about 3 month ago. and now my left eyes still got the double shadow. I've worried that I made a mistake when the laser work. Im sure that my eyes moved down when the green light is off.
Hopefully its not my problem then my eyes still on recovery progress.
Modern-day lasers have eye trackers that are faster than any inadvertent eye movement can be, so if you were operated with a modern laser, this cannot be the reason for a double shadow. This shadow could rather be due to prolonged healing, or something in the ablation profile "increased" your wavefront errors.

This was back in 2008, so I had LASIK with a mechanical blade for 7 diopters of myopia. Today, I would rather choose transPRK. LASIK is safe, but transPRK is even safer.

It's 500 microns, not millimetres! Are you aware that 500 millimetres is half a metre? I'm no surgeon but I can tell you that no one has a corneal thickness of half a metre.

What’s the probability of having permanent dry eyes
There always is a chance of dry eye after refractive laser surgery, but the risk is much smaller in PRK. In contrast to LASIK, the nerve endings are not cut by the flap, which is a driving force for dry eye.
