sir,in your 5th rule you said about concrete cover. it means clear cover right. you are saying about effective cover, isnt it?. because effective cover is between top steel center to top edge distance. so concrete cover and effective cover same?

max spacing of columns should not be more than 5.5 metres

How about determining the axial load for column sizing?
Calc the load transferring from slab to beams and from beams to column.
