He has a video on how to treat fibrosis... My question is when you say prevent it in the “early recovery” how long is early do you mean first 3 weeks, first 6 weeks, or first year post op?

YES, but what if you do have some fibrosis?? What can be done about that?? And why do some surgeons (mine) NOT warn you about that and advise you to WEAR A FAJA??

I've been researching and some doctors have said to do ultrasonic cavitation treatments, which is ultrasound, radio frequency waves that goes beneath the skin and breaks down tissues. I got full abdominal liposuction and wore the compression garments, did the massages, but that only goes so far. I bought my own ultrasonic device and started using it 2 to 3 times a week and am seeing some difference, so it may work for you. I started doing this one year after my surgery.
@@paulg6527 Today I am 8 months post op. My swelling has gone down a lot, but some areas are a bit hard like the flanks in my lower back side above my butt area. That still is swollen and hard at times. Wearing the fajas help me with that, but I bought a massager and am going to use it more often now that time has passed. I hope that as time passes I no longer have this problem. I don't have actual fibrosis, thank goodness! Thx for responding, I will look into that.
@@cherrylanemonroe1082 the hardness, to be honest, should've gone away after 8 months - mine did after 3 months. You may look a but swollen and that will subside, but within 6 mons you should see what the end results are. I would continue with the massages, deep tissue more or less, so that the swelling minimizes and all the little cavities/tunnels created by the cannula closes and scar tissue don't develop. I wish doctors would really stress about post op care more so that you get the results you want.
@@paulg6527 I am 51 almost 52. I heard that people my age don't heal quickly as younger people do. I am naturally thin and I look young, but my age is too old I think for having a TT & Lipo. I will continue with the massages, but a lot of the hardness has passed. How old are you? I bet you recovered quickly because of your age.
How do you treat it after you already have fibrosis
I asked that here, and this doctor's response was not satisfactory. I'm starting to think this is a major problem that nobody is talking about, much less taking seriously.

As a RN I would say go for it. Just had mine done June 5th. The recovery sucks but just make sure you have help…follow everything your doctor says
When it comes to lymphatic massages after BBL and you say 20 massages are better than 10. Let’s say you are getting your BBL from out of state and recovering there for a couple of weeks and you start your first massages right after surgery. Your massages are only 30 minutes long. Would doubling up on your massages each day, making them longer, count as 2 or do you mean 20 days in a row?

I wore a faja 23 hours a day with foam and did 12 massages before my fibrosis developed at between 3-4 weeks. I don't know if it can be prevented but I will continue getting massages to help break it up.
Fibrosis can't really be called fibrosis until at least 6 months as it is normal to be lumpy and bumpy initially. Are you rolling? You should. It is important. Here is a link if you are interested: ogeerecovery.com/collections/ogee-roller

Rolling is the most important way to prevent fibrosis and it's never too late to start and you can never roll too much.

I just had 360 lipo and bbl with dr.William . I am 1wk post op, I paid for my drainage massages because of the importance they are in your recovery and no one ever contacted me i kept asking for info and all the staff would say they will contact you. I called the number the front desk staff gave me and nothing they also provided an email for my refund and they still have not contacted me. I am not from Miami, I had to look for someone to try and schedule my massages. Very very disappointed and I’m still waiting for my refund.

Everyone is different I’d suggest you go to a pro on the matter and let them look at you and give fitting advice. There are several treatments to target fibroses but you have to be consistent

I was 30 had lipo on my side legs and under and i had zero cellulite my dads side or moms side never had it even in old age and that Dr made my legs look dimpled im 45 now and I'd love to fix this permitted. I have awesome genetics as far as skin I'm very healthy i dont smoke social drinker but thats on rare occasion and go in for regular check ups I'm just depressed over my legs 😢 im pretty fit. I was young and didn't educate myself.
What if you had lipo but was unable to prevent fibrosis? What would you do then?
There is not too much you can do. I am working on a few things but I don't have a large enough group of patients or experience to really say that it works yet.
Thanks so much for your input. I'm a medical massage therapist who has been doing many massages for post liposuction patients. My question is what are your thoughts on using a "Rapid Release" vibrational massage "gun." Two weeks post op...harmful or helpful. Thanks again!
In my opinion at 2 weeks after surgery the massage should be focused on light, vibrational therapy to act as a manual lymphatic massage to drain the fluid. I think something like a theragun can be good for spot areas.

That can be a little tricky but if you are unsure then I usually order an ultrasound.

When you say massage minimum 10, do you mean professional 10-20 professional massages? I had chin lipo.
Yes. Massages by a licensed experienced massage therapist. Someone who his also licensed in lymphatic massage is even better.

What about Ultrasound and/or Shock Wave? From seminars attended, there is talk about Ultrasound wave , red light to help with fibrosis, adding Steroid injections!

what about radiation? it's used to prevent queloids, wich is also an excess of colagen, can't radiation help prevent fibrosis?
You are correct that radiation treatment is used to prevent the development of keloids. However, we are using it in patients known to have keloids. For example, if a keloid is excised from a previous surgery then radiation therapy might be used to treat the incision shortly after surgery to try to prevent the recurrence of the keloid in the same area.
For patients undergoing liposuction you don't know if they will develop fibrosis or not. If you gave radiation therapy to everyone after liposuction you would be subjecting people who would never have developed fibrosis to needless radiation. The radiation for a keloid is a small area, but the dose of radiation to treat the whole trunk would be a dangerous dose of radiation and potentially induce cancer in people who weren't even at risk of developing fibrosis in the first place.

The "hipotetical" patient I have in mind is someone who does have one or a few keloids, from deeper cuts (not a mere scratch, pimple or piencing), that didn't grow over the years (a "mild" keloid problem, not a severe one). In that case, would it be a good ideia, even if only for areas more prone to keloids, such as the back?
I've also learned from professionals that some lasers are great for both preventing and treating keloids, the woman who treated mine said she had a client using it after lipo to help prevent exesive fibrosis. But I simply can't find ANY info on it...

No. Fibrosis can only be diagnosed after at least 6 months. Everyone is hard and swollen to begin with but most patients will get soft with good recovery care.

That's what I want to know.
It can be done and fat is amazing but I don't use fat for a breast lift because the results aren't was good as using an implant. Quite a few years ago the plastic surgery community including me was very excited about using fat for breast augmentation and breast lifts but the results have been disappointing and just can't compare them to a breast implant. Fat is useful however for combining with an implant during an augmentation for some patients.
@@DrWilliamMiami I just had liposuction on the 7th. And I feel super weird and uncomfortable movements in my stomach like if i were pregnant, and the baby was kicking. I feel tightening, and obviously pain. The clinic where i went completely has ignored my post op symptoms. Can you give any ideas?????

There is a surgery clinic in Coral Gables that does some sort of ultrasound treatment for after-lipo fibrosis. Could that be a solution?
I haven't seen any data to support use of ultrasound for fibrosis but it's a bad problem and patients are eager to try anything that may help.
It's been a month since my procedure will the fibrosis go away ? It's round and hard ...I'm doing everything the doctor mentioned, it is small but still there
We don't really usually call anything fibrosis at a month out because most people are still lump and bumpy at this point and fibrosis can really only be diagnosed after about six months. I would encourage you to roll with the Ogee roller to get the softest skin.
If someone is known to develop post-op fibrosis and scarring, is there a way to prevent it from happening like steroid injection directly in the wound soon after the surgery? Especially after a tummy-tuck? Thanks
Not really because you would have to inject the whole abdomen.
Hello I am 44 years old and have recently had a bbl about 4 years ago I had a breast lift and argumentation about 6months after my surgery I was feeling something wasn't quit right and began asking my surgeon some questions on why they felt stuck to chest cavity he waived me off several times so many times I began to feel like a nag I now have the snoopy boob and I hate my breast's. I won't even remove my shirt in front of hubby!!. I have recently had a bbl lipo360 and I'm feeling the same way I have developed a saromo now that I believe has turned fibrosis my rear has slightly changed though not much but my stomach lookes horrific it's bumpy and bulgy I looked better befor I started all this I'm not sure I should raise more concern I was told it will take time but that it possibly won't change much what I asked from my surgery was not done and I feel botched I like my sergun I don't feel anger towards her I'm just bumbed that something like this possibly happened twice and I don't know what to do plz some advice thank you I will send pics if u let me know where to send them ..sincerely - Shug
Massage or manual lymphatic drainage(MLD)?
It is advised to have 10 sessions of MLD following surgery to reduce edema and prevent fibrosis. Please clarify what kind d of bodywork helps the most as clients are confused about it.

As you are well aware, terminology is vitally important when discussing modalities with clients. A massage is a form of manipulation of the tissue in order to release tension, Whereas manual lymphatic drainage Is a specific modality that aids in the healing process edema and preventing fibrosis.

How many minutes per day do we use the massage roller? And is it over the faja and foams ?
So you can't roll too much so as much as you can comfortably fit rolling into your schedule. At the beginning right after surgery you are going to use it over your faja because you will be too sore to use it on your skin but as soon as you can start to use it directly on the skin. Roll twice a day for a whole 1 year after surgery for the very best results.

Is hyloronidase injections a good way to get rid of fibrosis?
No.
@DrWilliamMiami. Had lipo 3 months ago. Now i have several spots of very hard fibrosis because i did not follow aftercare instructions. I did not wear my garment or do massages or folow any prevention methods. A masage therapist recommended that I get hyloronidase injections to dissolve the fibrosis Matrix. Can you please make a video on how to get rid of fibrosis that has already formed.
You can't get rid of fibrosis. That's the problem. You have to prevent fibrosis because once it develops unfortunately there is no therapy for it.
@DrWilliamMiami will I have to live with it forever? It's very noticeable, hard and looks ugly. I wish I was educated on this topic beforehand.
I wish I could give you a different answer, but that is why we roll and use the faja and get massages and use FibroGuard etc. What happens is the fascia contracts and scars, and then that is reflected on the surface of the skin and there is no treatment to go in and make the fascia soft again.
What if my massages are spaced out and not back to back would I get fibrosis?
I think it is important to have the massages right after surgery to be everyday and then after the first week or ten days you can space the massages out to every other day.
I’m 5 days po liposuction to abdomen and flanks and I started my Lymphatic drainage massage next day after surgery. I hear people saying these massages shouldn’t be painful but mine has been painful is this normal? Also my stomach is very hypersensitive is this normal?
So I hear that the massages shouldn't hurt as well. But I don't understand how it is possible they don't hurt. I have spoken with several very knowledgable and well trained lymphatic massage therapists and they assure me they are not supposed to hurt. I understand why they say that because true manual lymphatic massages are by their very nature--opening up the lymphatic channels--gentle. So I get all of that.
But here's what doesn't make sense to me. I have operated on many liposuction patients over almost 20 years and I can tell you that liposuction hurts. No getting around that. It hurts. And massaging and touching someone the next day after liposuction no matter how gentle you are, or delicate you are just has to hurt, and from what patients tell me is that it does hurt. but that it feels much better after the massage as there is less swelling. Swelling itself hurts. I know personally if I don't wear my compression socks and stand operating all day my calves hurt from the swelling at the end of the day. So swelling itself hurts.
So here is how I make sense of things. I think that early massages after liposuction do hurt regardless of who is performing them because your whole trunk is badly bruised and swollen but it is the later massages (when I think the lymphatic massages should be performed) don't hurt because they are gentle (MLD Massages are gentle) but the early ones hurt because you are badly bruised.

Someone close is having a similar experience. I feel their pain and yours by extension.
I did all of this and still have it. Bad. On both of my sides. It feels like tight cords under my skin from my bra line to my hips. So painful. 5 months post op.
I’m 1 year 4 months and still tight and swollen in my upper abdomen I compressed 24 hours but it’s still there, I’m so over it
After one year still swelling 😬. I’m two weeks post op. I feel really tight. I pray you continue to heal and the swelling stops soon.
If I already have fibrosis and still performed all the recovery procedures. What else can be that can done to eliminate fibrosis?

Once you have fibrosis it never goes away. It is permanent.
I purchased your og roller I am 2 weeks post op. I plan to start using the roller at week 3. Should I use it directly on my skin , on top of a tank top or faja. I am still sore.
If you can tolerate it on your skin than do the skin. IF you are still a little too tender than begin on the faja. Skin is better. Roll 2 ways. First use is as a rolling pin to really work all of the areas of liposuction and then use it very lightly with vibration on and that acts like manual lymphatic drainage. Work it with the vibration down towards each groin.
