I am 18 months into my prostate cancer journey. Gleason was 9, so I opted for radical prostatectomy and removal of lymph nodes and any surrounding tissue that might be involved. My prostate tumor was around the urethra and so the neck of the bladder and some additional safety margins of the urethra outside the prostate were removed. I just had my 18 month PSA check and I remain less than .02. To me, this means that my surgeon 'got it all on site' and now I have to watch for biological recurrence at some point in the future. I will be committed to PSA check every 18 months, but most of the literature I have read says to me that at some point it will come back. I was blessed with complete incontinence as a result of the surgery, but through a great physical therapist, I now have about what I would call 70% control. One day at a time.
Hi Im also a gleason 9 and had ralp last november and waiting on my first psa test 60 days after surgery which I will have tomorrow. Like you my incontinence is really bad, basically at night Im "ok" but walking around Ill ho through a pad every 30 mins so thats tough but frankly its better than a gleason 9 tumor inside me running havoc, so Im grateful albeit struggling, we will see how the future looks and if it improves. I wanted to ask about your sexual function if I can? Im 54, 60 days in nothing but they say 6 months..Ill see, cheers
I had 9 months of physical therapy to deal with the incontinence, and I reached the point of being able to control the outright flow of urine. Could not stop the drips, but the flow was controlled. Find a pelvic floor physical therapist. I found a young woman (about 30) who was amazingly helpful and encouraging. As to sexual function, I was able only to achieve an erection with injections of 'trimix' directly into the penis, and it was very unsatisfying. I am 75, married 52 years to the same lady, and it is not an essential part of our relationship any longer. We have achieved such a depth of emotional intimacy that the physical was literally no longer essential. Of course, we only found that out as a re sult of the cancer, but it is true. The injections were about $25 a pop, and we just agreed not to bother with them. The whole process was a bit dehumanizing anyway. But at 54, we were still very active sexually. Ask your urologist about the shots and the ways in which you can insure that when the time comes, you have not atrophied down there. Blessings to you - Tom Nelson - @@nelsonoliva2119

I have Stage IV Prostate Cancer with Bone Mets. Tried the ADT, but my Liver couldn't take it. I tried chemo, but had a terrible reaction to it. I had tumors pressing on my spinal cord and had surgery. I'm learning to walk again. Lots of pain.
Thanks for the information. I was diagnosed with prostate cancer G7(4+3). I have gone through 38 days of radiation, 5 times a week I have also had 2 injections so far of hormone therapy. My problem now is that l still visit the loo about five times each night. My PSA is 0.5 .

This was very helpful! I have yet to be diagnosed with prostate cancer. My free PSA was just below 10 a few weeks ago and I am going to have another PSA test in a couple of weeks.
This is quite reassuring as I’m having my first biopsy done tomorrow after my PSA came up 5.2 and MRI scan presented something although my urologist hasn’t divulged any more info. I also did a second PSA and wasn’t told the results either although I guess I’ll find out when I attend the biopsy. Worrying times.

Pallistive and home nurse say I should be looking for full time care and hospice. oxycodone every four hours and morphine every 8 hours. At 76 does this cancer just take you faster?
Mar2 2023 went to the er thinking I had pinched nerve or slipped disc. Mar 4 diagnosed stage 4 prostate cancer that had metastasized to most of my bones. Psa was 603 had radiation on one tumor that was pinching the spinal cord. Started hormone therapy after a week or so and now get lupron once every three months and Xtandi daily. The treatment may kill me before the cancer but I’ll give it a try. Psa dropped to 79 after a month and now down to 5 after 2 months

@@schmingusssno it failed after holding my psa at 8 to 10 for about 9 months. I’m currently taking pluvicto two treatments in but psa continues to spike currently 569. Didn’t have anxiety with adt but definitely increased mood swings. Good luck to you ❤

Charles, I hear you brother. Hopefully your biopsys continue to show nothing. Not so for me. Last January, even though I had no symtoms other than the usual middle-aged "bad back" ( which we all get one way or anonther, right?) I had a PSA test that came in at 97.2! Whew. Next the biopsy showed a gleason of 8. Then the scans. Heck, I had absolutely no idea...my pelvis was already fractured and it had also spread to a lymph node. My wife and I talked about it and we agreed, becuase it was obviously incurable, not to take treatment as such and let nature and God take their course. Now I'm in hospice care and it's okay. The pain is managed fairly well and I must say having a living Faith is a huge consolation to the whole family. Even my littlest, age 12, seems to know that daddy might be with Jesus by Christmas. So, again, I hope that the doctors can bring your PSA down and, if it's alright with you, I'll say a prayer for that intention. Cheers, my friend.

Gleason 6-- no. But the only way to know the grade for absolute certainty is through surgical pathology. There could be higher grade missed in the original biopsy, or the cancer is found to be of a higher grade than what was initially reported in the biopsy

Hi, I was diagnosed Oct 2022. Grade9 contained, I was diagnosed sept 2023 with bone mx multiplied sites. I've been told it's very aggressive and a rare mutation which actually attacks my bones making then thin rather than building excess on top. My pain levels are low and controlled with paracetamol my condition is being treated with injected hormones and abiraterone. My psa went from 16 to 37 in for weeks, but on my last test+4 weeks it's raised to 40. I won't lie the speed of progress is very scary, I'm only 59.
I'm 58 diagnosed at 57. Are those PSA levels whole number values? If yes I understand the concern. I had a Gleason 4+4 Grade 8 an aggressive type. PSA went from 7 to 8 within 2 weeks. They thought it was contained but there was some migration, luckily it remained in the pelvic bed and didn't get to the lymph nodes. I'm good now 1 1/2 yrs later having had prostatectomy and had my last hormone shot about 3 weeks ago. Hang in there man, I know it isn't easy but live life to the fullest and stay strong best you can. God hears us but he picks the right moment to answer...kinda inconvenient at times but he has his reasons I guess.

I hv height risk Prostate Cancer (T3b.GS5+5 & PSA 61.8) I hv started on Degarelix from 17/9/19 with good respond - after 12 monthly Degarelix my PSA now at 0.111. My side effect hv been swelling & pain for up to 1 or 2 weeks / sometimes fever. Recently my Urologist stop my Hormone for 6 mths - then go back & check my PSA again. My Question is how Long can I continue my Hormone Injection & any any other medication like (Dupoint or Casidex) can help. I was told Hormone cannot cure Cancer ? What is best for my case. Prefer not to go Radiotherapy- Thank you
As usual, very informative. Here's a question. What pray tell is advan e cancer? My Gleason is 3+4, Grade 2 with one leasion and a PSA of 9.3. After Lupron and Orgovyx 150 mg. Yet in communication the Oncologist refers to advance cancer.
My dad is 73 years old. He has developed prostate enlarged. MRI reveals Pirads-2 nodules in the transitional zone. PSA shows 16.57* But after the second time test shows 10.700 with Psa free 1.900 and 18% Here, should he go for a biopsy again ?

I had a Laser surgery on my Prostate about a year ago, I don't go to the bathroom at night only once. But during the day I pee frequently. Is that a good sign, that my Prostate is not that bad. Waiting for a reply

After being told by a UROLOGIST over five years that my prostate is cancerous here am I with the only treatment seamiñly available is the change of CAPHITER
In 2011 I had a 12-needle biopsy. Only 1 sample showed cancer. I was Gleason 6, 3+3. A follow-up 12-needle biopsy showed no cancer. Great. Apparently, any cancer I have is very small. My question is, once a man is a Gleason 6, is he always a 6? Don't men who have higher Gleason scores start out at a 6? Yes, their biopsy might show them to have a Gleason score of 8 or higher, but were they once a 6 and just didn't get a biopsy at that time?

Is it unusual for your urologist-oncologist NOT to tell you your Gleason score? Ours has not (despite us asking). We're on Firmagon/Eligard (1st injections each thus far), opting for hormonal therapy for a newly diagnosed 91yo male patient with possible metastasis (bone scan next week) in otherwise fairly unremarkable health.
Not clear that I understand what PC is Fatal and what is not. I get alot of PC is not fatal. but what PC is fatal. Do not have enough information to answer the 6 questions. I have GL 9 metastic cancer into Lymph Nodes. So is it serious or not? My research says I have a 20% chance of survival in 5 years.
That’s very serious. Need to take action to cure it . It’s a process to fight with cancer. Change your life style and find a good doctor first. Hope is there. Curable but need to fight for it.
I believe it depends on your psa number. In less than a year, I went from .03 to 4.0 and the doctor ordered blood tests twice a year. I did have a blood test one month after the 4.0 results, and it showed
biopsy so-called negative with a high grade pin 12-2015, then in 10-2016 tumor 4.4mm, out of prostate so says the MRI, Gleason 8, 5+4. slow growing my ass.

Did you try Lu PSMA therapy for advanced Prostate cancer? It is supposed to be very effective and with very few side effects.

What about a PSA of 45 Yes 45. I’ve had the first biopsy with 12 leads. Nothing found, I had the targeted mri, and the 2nd biopsy nothing found.

My Question is how Long can I continue my Hormone Injection & any any other medication like (Dupoint or Casidex) can help. I was told Hormone cannot cure Cancer ? What is best for my case. As usual, very informative. Here's a question. What pray tell is advan e cancer? My Gleason is 3+4, Grade 2 with one leasion and a PSA of 9.3. After Lupron and Orgovyx 150 mg. Yet in communication the Oncologist refers to advance cancer.

MRI reveals Pirads-2 nodules in the transitional zone.
PSA shows 16.57*
But after the second time test shows 10.700 with
Psa free 1.900 and 18%
Here, should he go for a biopsy again ?

Yes prostate cancer killed my father!!!! Treat it aggressively. My dad was emaciated when he died. It was horrible to watch.

I am 74 yr old male chinese. My PSÀ over past 6 mths range from 3.9 to 4.3. MRI concluded i have Pirad 5. My urologist decided i need biopsy. Wd u agree to doing biopsy or repeat PSA test before doing biopsy? Your opinion is appreciated. Thank you.

In 2011 I had a 12-needle biopsy. Only 1 sample showed cancer. I was Gleason 6, 3+3. A follow-up 12-needle biopsy showed no cancer. Great. Apparently, any cancer I have is very small. My question is, once a man is a Gleason 6, is he always a 6? Don't men who have higher Gleason scores start out at a 6? Yes, their biopsy might show them to have a Gleason score of 8 or higher, but were they once a 6 and just didn't get a biopsy at that time?

@@kathleenking47 The doctor uses the numbers 1 to 5 to grade the most common (primary) and second most common (secondary) patterns of cells found in a tissue sample:
Grade 1. The cells look very much like normal prostate cells.
Grades 2-4. Cells that score lower look closest to normal and represent a less aggressive cancer. Those that score higher look the furthest from normal and will probably grow faster.
Grade 5. Most cells look very different from normal.
The two grades added together are your Gleason score. Cancers will score 6 or more. A score of 7 means the cancer is intermediate, and a higher score (8 to 10) means the cancer is more likely to grow and spread.

10-15-20 years living on androgen deprivation therapy.....sounds peachy.

Is prostate cancer fatal? Ask all the thousands of men who died from it or who surely will die from it in the future. Are prostate cancer treatments fatal? Yes, it is just a bit more time waiting for the end.

Is it unusual for your urologist-oncologist NOT to tell you your Gleason score? Ours has not (despite us asking). We're on Firmagon/Eligard (1st injections each thus far), opting for hormonal therapy for a newly diagnosed 91yo male patient with possible metastasis (bone scan next week) in otherwise fairly unremarkable health.
Not clear that I understand what PC is Fatal and what is not. I get alot of PC is not fatal. but what PC is fatal. Do not have enough information to answer the 6 questions. I have GL 9 metastic cancer into Lymph Nodes. So is it serious or not? My research says I have a 20% chance of survival in 5 years.
That’s very serious. Need to take action to cure it . It’s a process to fight with cancer. Change your life style and find a good doctor first. Hope is there. Curable but need to fight for it.

biopsy so-called negative with a high grade pin 12-2015, then in 10-2016 tumor 4.4mm, out of prostate so says the MRI, Gleason 8, 5+4. slow growing my ass.
