The image is from chapter 90.
Guts wakes up to a changed body, he's missing an arm and an eye, causing immense physical pain. Despite this, his first concern is Casca's whereabouts. However, he soon realizes that Casca has transformed, she doesn't even remember who he is anymore. At first he denies it, clinging to hope that she will remember him. When Casca bites his hand, the emotional pain and confusion hits him hard. Casca rejects Guts and seeks comfort from Erica, which shatters his hopes and plunges him into denial and despair.
Overwhelmed by the horrors he has witnessed, Guts attempts to escape the unbearable nightmare he has awakened to. Fleeing physically, he seeks distance from the torment that has become too much to bear. Yet, even as he runs, his mind is haunted by traumatic memories and the painful loss of his friends. As he continues to run his bandages unravel which symbolize the loss of his comrades and the betrayal of Griffith, as his wounds reopen, bleed and remain unhealed.
As Guts continues running, he gradually realizes that escaping doesn't bring him the relief he seeks from his emotional turmoil. Fatigue begins to take its toll, and he eventually collapses into the tall grass. This act symbolizes his surrender and his growing willingness to confront his struggles instead of running away from them. Isolating yourself can be damaging and it's more beneficial to face challenges and seek support from others.
Under the moonlit sky, Guts has a moment of clarity. As he looks up at the starry sky and sees Femto, the raindrops falling on him reflect his tears. Femto, the embodiment of Griffith represents the lasting impact of their shared history. It signifies the lingering anger, resentment, and desire for revenge that Guts still carries within him.
This encounter with Femto also highlights Guts' ongoing struggle to process his emotions and move forward. Despite being apart, Femto's existence shows that Griffith's influence and their shared traumas still affect Guts' life. It signifies that Guts needs more time to fully come to terms with and overcome the profound impact of these traumas. This highlights how important it is for you to fully address and resolve conflicts in order to prevent them from resurfacing.
This scene serves as a significant moment in Guts' character development. It emphasizes his recognition of the darkness within him, his growing willingness to confront his struggles, and his journey to seek resolution and redemption. These themes align with the broader narrative of Berserk, where characters grapple with their inner demons and navigate the complex and brutal world in search of healing and purpose.

After GUts theme became a meme the over-saturation of it kind of ruined it for me TBH. After that, this song became the one I listen to the most afterwords.
