This scene is the first one in the series to take place off island in current time. Everything off island up to this point has been flashbacks.

Quarantine is not a group of people. It is a way to prevent the spread of disease. People suspected of being ill were grouped together in a closed area for 40 days (hence the name quarantine). And if they didn't develop symptoms, they could leave the area. Rudimentary but quite effective.
Everyone must follow Locke when he wants to push the button and everyone must also follow him when he no longer wants to. Ah! men of faith. Selfish. For some of them.
