Is all this just information based on pilocarpine drops? I was prescribed pilocarpine to help with dry mouth/eye symptoms. Should I be concerned of doing damage to my eyes

Thank you very much Dr. Richardson! These videos are excellent. Would love to see a video covering the different IOL options for CE patients, and some of the IOLs you’ve had success with!

Appreciate that. Timolol and Travoprost can be a very effective combination. Hoping that is all you will ever need.

I was prescribed Pilocarpine drops prior to cataract surgery and told to use it after. I've been using it for 2 years now, one drop in my right eye. Why is the use of pilocarpine necessary for eye pressure after catract surgery? Doesn't the replacement of the lens fix pressure issues?

I would like your opinion...3 years ago I had cataract surgery and I was seeing halo's around lights at night (very annoying) the surgeon told me that 1 in every 10 people have larger pupils and when he put the new lens in my eye the lens was slightly smaller than my pupil and that's was caused the halo's. To fix the problem, he put me on pilocarpine drops, and it worked...pupils got smaller and the halo's went away. I only use the drops when I am going to be driving at night. I really don't have any side effects, but sometimes the drops kinda mess up your eyes, redness etc. What do you think of this whole process? (drops are 1%)

Hello Patrick. It is true that some people have pupils that enlarge beyond the edge of the intraocular lens (IOL) at night. There are ways to detect this prior to surgery and I measure both photopic (bright light) and mesopic (low light) pupil size in all of my patients considering cataract surgery. However, many surgeons do not perform this measurement as to obtain it accurately requires investment in technology that can be expensive (i.e., 10s of thousands of dollars)--though a rough estimate can be made in the exam room with only a $10 penlight. In the rare case in which the pupil expands beyond 6mm (the typical IOL diameter), a special IOL can be ordered prior to surgery with a larger diameter in order to reduce nocturnal (night time) halos. In any case, the treatment you are receiving with low-dose pilocarpine is perfectly reasonable given that your IOL is already in place.
