Well going back to the Travelers Trilogy: Mission One (Part 3) Mangle was killed by Glitchtrap, my guess is either he or The Voice placed the fragment inside her while Willow was repairing her and it slowly infected Mangle from the inside out for the last two weeks, and poor Mangle never realized it until now.

At least you have a new nightmare that's an ally but also I see what you did with mangle now you went with the main protagonist of that one anime who swallowed a demon finger and he needs to swallow all 10 of them so that demon can die alongside him in his body but still at least you got a new nightmare that might be your ally and possibly her little troublemaker kids.
