There’s a death so rare that minecraft didn’t even know was possible. It involved someone building a nether portal, and guess what? That nether portal LITERALLY SPAWNED ON TOP OF ANOTHER PLAYER, KILLING THEM.
Actually it's how the game's coded, idk how to explain it to you, but each type of death have a template text, something like: [Victim] was slain by [Perpetrator], and when you combine those with something ridiculously impossible to get like killjng yourself, the game interprets that death to a death message using the template text.
For the death.fell.accident.water message, you just have to be at like 1,5 hearts and then throw an ender pearl beneath you, while you stand in water (that's how I and my friend got it on my friends server on 1.21.11 (it was an accident while we to were fighting for fun))

It’s quite common for Minecraft videos to be faked, and while I won’t say that this is it’s also common for me to kinda hope it was faked and that someone didn’t spend 3 weeks to fly there and back only to have 5 hours of it not working. (To be clear I have nothing against faked videos, it’s still a good video if some parts were faked, it’s just an over the top and kinda sad video if none of it was faked. You can set up timers but elytra with unbreaking 3 break in about 30 minutes, except if you’re unlucky they can break in 10 minutes so you’d have to pay attention and RUclips mobile won’t let me wind back but I assume they flew at -2.4 which IIRC you fly at about 15 blocks per second which is about 70 hours, 70 hours which is about 139 unbreaking 3 elytras and so that’s 23 hours where you’re safe and 47 hours where he needed to pay attention to potential breakages - it’s very much possible, but it’s not really worthwhile. The video showed a bunch of elytras breaking, not 139, but certainly enough for more effort that I’d be bothered to do)

This happened when the nether update released, i didnt know how to use enderpearls properly and i was getting attacked by a hoglin and i had low health i tried to use an ender pearl and ended up hitting a netherack ceiling i had half a heart and fell down into the massive lava pool below and then ended up getting the death message. I only remember seeing something like “blah blah fell from a high place and died to surface of lava”

it was most likely just "(your name) fell from a high place while swimming in lava" because lava doesnt cancel fall damage

You should have set up a stasis chamber at the spawn as well so you can just teleport back. Then you would’ve only had to do the trip once
