i think what he means is that using his powers to cause large scale destruction like that mansion is difficult for him. And it gets harder if there's vectors (objects, people) that he needs to take care of at the same time. So what he means is that it was hard for him to destroy the mansion but also make sure Utahime wasn't harmed by it. And we can see she's in the center of the destruction and there's not a single scratch on her..which proves Gojo made sure to momentarily put his infinity around Utahime to keep her safe as he destroyed the entire mansion. Idk why some female characters in JJK is look cute and happy when they were young, but look sad and tired when they are adult. I mean look at utahime and shoko, Utahime facial expressions is more lively when she was still a student. And Shoko looks always tired when she becomes the school doctor. That usually happens when you have to care for others. Utahime and Shoko are basically support characters, so it's to be expected. Mei Mei is all about herself, so of course she would look more lively.
adding on to the other commenter: uta is a teacher and shoko is a doctor. both of them had experienced loss of their friends ever since they were young, and now with their professions that basically deals with other people, there’s still a sense of fear that comes with the responsibility of taking care of people especially in their world. uta is a teacher that has to teach her incredibly young students how to properly do things safe and secure or else they’ll die (which basically has happened already) and shoko has to tend to injured people and even the dead. even if they are used to it, repeatedly seeing and knowing that the people you know are in constant danger is unimaginable and will definitely stress you out. that’s why their bad habits become a sense of coping mechanism too: shoko with her smoking and utahime with her drinking.

That usually happens when you have to care for others. Utahime and Shoko are basically support characters, so it's to be expected. Mei Mei is all about herself, so of course she would look more lively.
adding on to the other commenter: uta is a teacher and shoko is a doctor. both of them had experienced loss of their friends ever since they were young, and now with their professions that basically deals with other people, there’s still a sense of fear that comes with the responsibility of taking care of people especially in their world. uta is a teacher that has to teach her incredibly young students how to properly do things safe and secure or else they’ll die (which basically has happened already) and shoko has to tend to injured people and even the dead. even if they are used to it, repeatedly seeing and knowing that the people you know are in constant danger is unimaginable and will definitely stress you out. that’s why their bad habits become a sense of coping mechanism too: shoko with her smoking and utahime with her drinking.
