    Do you do Skype appointments? You are easily one of the most well educated, balanced and thorough practitioners I have ever heard. I would pay dollars to have an appointment

    Great video! Very comprehensive!
    Can gallstones be caused by elevated D-lactate, leaky gut and/or SIBO or vice versa? Can you recommend the safest dissolution therapy to safely dissolve and expel a stuck gallstone lodge in the bile duct? For example, bile acids/salts?

    I had been keto for 15 years. Leaky gut, sibo and your videos made me stop. Now I feel better and back to keto. I cannot tolerate constantly being hungry, not feeling full until I stuff myself and wanting to nap at 2.
    I am taking your advice about diversity and will carb reload on the weekend with high fiber foods. Like gmo shredded wheat and hormone milk.
    Also I did have d-lactate acidosis. It felt like I think autism would feel like.

    Thanks for the informative video. It was very insightful yet at the same time a little depressing. You sort of down played the d--lactate factor a bit and your biggest solution is to repair leaky gut. I've been working on repairing my leaky gut for 3-4 years and my brain fog hasn't really improved much. Repairing leaky gut seems to be like the climbing mount Everest so I tend to cringe every time I hear a health practitioner or doctor say that's your problem. Leaky gut seems to be most people's problems that have stuff like chronic fatigue or IBD or bad brain fog. I honestly haven't met anyone w/ serious illnesses who's really done it and I've been in the biohacking world for awhile. I do appreciate you saying to explore the liver/kidney functions. Do you know if there's a test to see if your kidney's are producing the enzyme D-2-hydroxy-acid dehydrogenase? If I can't fix the gut, maybe I can reduce the brain fog by supplementing w/ an enzyme or somehow get my kidneys to produce more enzyme D-2-hydroxy-acid dehydrogenase? Thanks in advance if you would be so kind to answer this.

      Hey, Fortmaker. I'm sorry you're frustrated... but that doesn't change (what I currently) believe to be the truth of the matter, which is that D-lactic acidosis is generally over-hyped. "Healing the gut" can mean different things for everybody, and it can take years. I wish I had better advice other than never give up and stay positive in any way possible. I was there for a long time too- living on "leaky gut repair" products and trying to heal my gut to no avail...
      To the best of my knowledge, there is no test for kidney or liver D-2HD. The closest we have is a urine measurement of D-lactate, which is available from Mayo clinic and can be ordered (probably at a high cost) through Labcorp.
      All that being said, brain-fog isn't a pathology in and of itself, it is a symptom that can come from many sources. Vitamin deficiency (namely B12, Iron, B6), inflammatory chemicals (cytokines) made by the immune system, thyroid dysfunction, and viral infections like EBV are all common examples I see in my office. Of course, those inflammatory cytokines can be made because of gut dysfunction and SIBO, but that's not the only cause of this symptom. I hope this all helped?

      Yes, your response is helpful, and your video was also very helpful. Thank you so much! Sorry for being somewhat negative, it's such years of frustration and every rabbit hole seems to have a dead end. I was curious if you have any videos online posted on how repair a leaky gut? Or if this is proprietary information, how much do you charge for setting up a protocol? I've done gut testing, read a ton of articles on the subject so I don't need a ton of help, just an hour or so to explain a protocol that might fit my condition. Right now I'm considering going on MegaSporeBiotics (or something like this) as I had a lot of people tell me it's helpful in sealing the wall junctions to calm inflammation and thus allow body to heal. Also, I'm curious if healing a leaky gut is even possible without curing hashimoto's first? I'm at that weird point where I have hypothyroid symptoms but according to western doctors my TSH score isn't so bad that they're willing to put me on thyroid medication even though my T3 is in toilet. Anyway, I hope to hear back from you because I think you might be able to help....if it's not too
      costly I'm game.

    Lol omg I found this video just after reading both of those papers and am glad to hear them re-digested out loud by an actual professional. I’m trying to heal myself (seems like straight leaky gut) and my coworker (more sibo symptoms than me). I look forward to poring through all these videos. Easily the most informative video I’ve seen in a long time. Thank you !

    Wouldn't RIFAXIMINE help to kill the bacteria producing D lactate?
    I took a lot of planctarum probiotics, I thought I was doing good, I took 3 times what was recomended for a week until I started to feel bad
    that was after finishing a rifaximine treatment, so the bacteria had my stomach for themeselves :( please help

      @yoyo I was getting better then I drank fruit water or something else, I'm not even sure what, and started to feel bad again
      I found a paper about using rhamnosus to clear the planctarum but I'm feeling even worse
      I think we just need to stop all probiotics and let it clear your system, it's very very slow
      the first time I was feeling well, I'd say the improvement was 1% a day
      so after a whole month of low carb diet, zero probiotics and lots of water, you should notice results

      The most important issue with most SIBO cases is impaired MMC function. Taking a prokinetic like rifaximine works mich better than antibiotics

    You recommend NAC. Can I ask you have you heard about sulfur intolerance and how does that play a role in sibo and is there nutrients that someone is missing that would utilize sulfur. When I take NAC I get egg smelling gas I don't get any other symptoms other than that so not for sure about sulfur intolerance. Also what if you take B1 and it makes you even more tired.

      Hmmmm. When the new H2S SIBO test comes out you might consider getting that done- that's the first thing that comes to mind. You can also get some stool testing done and see if you have an abundance of H2S producing bacteria. Depending on which scenario it is, it would be addressed in different ways.
      Also- people can also be Sulfur intolerant because of a genetic SNP called CBS, but that is not what you are describing (the presentation is completely different). I wanted to let you know- don't bother falling down the SNP rabbit hole for this one :)

    the genova diagnostics OAT does show D-lactate whereas the great plains only shows L-lactate

    The problem with some of the logic here is that its not considering how many are getting D Lactic acidosis with just otc probiotics. We are putting probiotics at the top of the GI tract where they really shouldn't be. I had this without short bowel, roux en y, or anything else associated. The only lead we had was probiotics. So while everyone keeps saying add "diversity" with probiotics and add prebiotics for the "good" bacteria, I literally can't do either. Its caused several vitamin deficiencies, muscle pain, and horrible fatigue and brain fog. I also can't have any fiber without bloating and gas.

      Do you now take non-lactic acid forming bacteria? I'm not taking any right now as having a stool test soon. But then will only be taking non-lactic acid forming bacteria. (Bifido, and I think soil/spore based bacteria, from my initial research so far).

    Once the bacteria have been cleared out. I can't tolerate any kind of Lactobacillus strains,will flare up symptoms.Do you have a suggestion for a probiotic that won't cause d lactate issues?

      There are many options still! Bifidobacteria strains should be safe, as well as the D-Lactate free probiotic from Custom Probiotics.

    If someone has Lyme and coinfections but feels totally normal abd healthy when the gut is doing well, do you think they should focus specifically on the gut or specifically on the Lyme and co?
