And the way this game work is if you kill an innocent, your character will instantly die regardless of what health value they had. They died because they killed an innocent, Which resulted in an immediate game failure.
What happened was Frank killing the guy reset the universe, and he carried on in the new one where he was retroactively turned into a criminal whilst us, the audience, remain in the original now closed universe.
Fun fact: There's actually an option to spare the guy your holding as a shield by hitting triangle, Y, or whatever button on PC. Doing so will make Frank hit the guy in the head with his gun. This is actually confirmed in game when doing so to the guys who give you the PTSD flashbacks as Frank will give a line of dialogue implying their survival.

"Look at one of the developers dads at a miscellaneous function!" This game was honestly really fun. Janky and simplistic, but very fun. They could reboot this game nowdays and it would be very good indeed, rework a few plot points, make the levels more varied, yknow, but the power fantasy of "kill awful people in the worst way possible" is such a draw. The insanity of the source material combined with the entertaining jank of this game really brought out the best of the boys in this one.

I'm rewatching this after watching them play Dead to Rights for the first time. I thought that game was so similar for some reason. They were both completely stupid and "bad," but I loved watching the Zaibatsu play it. Both games feel like they should have ended far sooner than they did. I would have to say Punisher is much less frustrating when comparing the two games stories.
