I have a question....I already know the concept and mechanism of this....but one question keeps on irritating me and confusing me... When we are alkalinizing urine with sodium bicarbonate, bicarbonate is trapping aspirin in tubules (refering to ion trapping process) and so aspirin is excreted....but my question is why sodium bicarbonate is not neutralising aspirin IN BLOOD when both are already present in blood? Why it has to work in urine specifically??
The absorption doesnt only depend on whether the drug is basic or acidic. It depends on many other factors as well, gut lining, particle size route of administration etc. Just it may augment in the process of absorption.
