I just assumed that pure blood heartless form by those consumed by darkness . As for the artificial heartless I imagined that the general rule of thumb was that if emblem heartless steal the heart of a person you get another emblem heartless. While if a pure blood heartless were to do the same you would get another pure blood. As for boss level heartless of ether types , they likely would form from strong hearts that were turned or were lower level heartless at some point who grow/evolved as they stole more hearts. It would be a good way to explain boss level heartless like guard armor, behemoth, dark side, shadow sneak and so on.

That seems like the likely explanation since that guy in KH1 turned into a Soldier Heartless before they were introduced as regular enemies.
