Because the only way you're getting a Quality 4 item is if both items are Q4, that means that you're going to have an extremely high chance to be forced to pass up god items like Brimstone.
Will it be difficult? Probably not, even Q3s are pretty good. But I know it'll be excruciating to look at Sacred Heart and have to walk past it.

I just got a run whith jacob and esau where i got hush, mega stan and delirium thanks to a random magic mushroom in the secret room. and i only went for the magic mushroom play because i always watch your videos. I think this challenge would work better with the Visible Options! mod, so could tell when you actually need to make a choice

True mattman moment is begging for death certificate, but not realizing he would have to take the worst item in the game.
Decided to do some isaac runs thanks to these videos, just had a pretty fun run, started as Lazarus and walked into the curse room, immediately got guppy's head and cursed eye. Decided to roll with it and ended up heading to the item room, and it was ipecac.
Eventually got guppy transformation and things went wild from there. Managed to get to and beat satan with few issues. It got to a point where I just picked up curse of the tower just to see if I could win before I died. Ended up unlocking quite a lot, though was stopped at satan since I didn't have the negative unlocked yet.
this is like rolling with disadvantage in dnd except its EVERY roll

Hi Matt man hope you see this I have an idea for a video you start with every quality four but every chapter it decreases in quality so on basement one to two you have all the quality fours caves you have all the quality threes and bye chest or darkroom you’ll have all quality zeros (you can just pick an active item)

I’ve been trying to find one of mattmans videos but I can’t seem to find it. Does anyone remember that video where the game glitches on Matt and as he was walking between rooms normally instead of walking into the next room he walks into the I am error room if I remember correctly. I think i remember him saying the whole video that the game was acting weird and he had no idea what was going on with it, and then when that happened he just lost it.

Angel deals can have choice pedestals while devil deals are usually a choice because health is a resource, but every item is available in devil deals

Hmm, okay, variant of this idea. Each time you find an item, subtract its quality by one. So NO quality 4 items whatsoever and any good items could potentially be reduced to garbage. Now THAT'S true pain

You should do this challenge with lost/t.lost and damocles, by forcing yourself to take only the bad item always, i think it would be very interesting with normal lost
