I was once diagnosed with chondromalacia patallae, caused not by misaligned tracking of the knee cap but by "lazy" (slow) tracking, due to misfiring of the quads caused by a "slack" rectus femoris; at least that's how the physiotherapist explained it to me. The doc felt the roughness under the knee cap by rubbing it firmly from side to side while my leg was flat on the table. Luckily for me, I was pain-free in about week after doing straight leg raises with a few pounds strapped to my foot.
Prior to being told about the leg raises, another physio diagnosed patella tendonitis (the pain is usually in the same area) and told me to rest, ice and stretch, but that treatment did nothing after a month.
I could be wrong on the following, but: I suspect it's better doing the leg raises while lying down; in other words, starting with the leg lined up with the upper body prior to raising it. Either that, or standing and kicking out from a weighted wall pulley that's close to the floor. I guess that doing the movement this way doesn't tighten the hip flexors too much, whereas standing and kicking up passed 90 degrees might, due to the extra hip flexion.
My injury occurred about 25 years ago, and sneaks back in minor way every few years, at which time it quickly goes away after doing the leg raises again. Yes, I probably should do them all the time, but, well, you know...life sometimes gets in the way. :-)
In my humble, uneducated opinion, the "regular" chondromalacia that I had gets overlooked too often in favour of misalignment (PTFS), and I'm not sure why.

Thank you, Bob an Brad! I’m a 13 year old girl soccer player, and it turned out i have chondromalacia and soft cartilage in my right knee, and i’m gonna do physical therapy for a while. I’m gonna call the man who owns the gym in my hometown, Tvøroyri, and if he don’t know what i can do for my knee pain, i’ll have to go to the doctor AGAIN. I’ve been at the doctor’s office 3 times, the hospital 2 times, and 3 different doctors with my knee. I practice soccer 5 to 6 times a week. I know i shouldn’t practice so much, but soccer is my everything. My friends, family and doctors keep telling me that i shouldn’t practice so much right now, bcz i just came back on the field after exactly 6 weeks of being BANNED from playing. I love soccer with all my heart, but i’m in so much pain when i play. I may need to go to the hospital to do physical therapy, if it turnes out he can’t do anything to help me. I can go to another town to get physio, but then i’ll lose many hours at school.

I wish I could watch the whole video, but when the doctor in the blue shirt started to move the knee back and forward I've had to stop. My last MRI was in 2010 , and I've got Chondromalacia of the Patella with subchondral marrow oedema in the femoral trochlea I haven't reached 40 yrs as yet. I was a gymnast. My knee problems started around the age of 8 doing classical ballet. In order to strengthen the thigh muscles, I went on gymnastics. By the age of 15 my knees were killing me, and I've had to stop gymnastics. I did not stop being active and started dancing, and by the age of 18, I could hardly walk, drive a manual car, and it was then that I've had the Keyhole knee operation which didn't fix anything. In 2004 plantar fasciitis was added to the mix, exercise is very hard for me. My brain wants too, but my legs are telling me no. My knees will give away at anytime, going up a step, doing down a step, getting up from sitting down for a while. The amount of times I've almost fall is crazy e.g. when entering a bus (public transport), as the first step on the bus as is high, like going up a step. The bus drivers always look at me with big eyes, like what is wrong with this young lady? Not knowing my knees are of a 80yrs old lady (one with bad bones that is).
Naproxen and codeine are my 'friends', McConnell taping did work for me. Nevertheless like I've been doing for years, is just leaving with these pulsating, unreliable, locking, give away broken knees, as they will not kill me.

I'm an OT. I had an MRI recently that said I had mild chondromalacia on the medial aspect of my right patella and a slightly lateral malalignment of my left patella with mild pain. My doctor still thinks I have a tiny medial meniscus tear even though the MRI said my meniscus was fine. He only thinks it's that because "mild chondromalacia does not cause pain" and because my knees get irritated when someone bends and twists them. Well...I think that will cause knee pain with anyone with kneecap issues. Plus I have mild athritis in my back that causes some pain, so if something mild in my body can cause me pain, I don't see why mild chondromalacia wouldn't cause me pain as well. Going to PT soon to get some help.

Would be good to consider/discuss that we don´t always move/bend our legs in the "perfect scenario", i.e straight line and to the front (which would provide perfect tracking of the patella, in theory at least). Consider soccer movements: tons of out of balance, multiple direction, one leg stands, through a game. Anyway, to anyone reading: stretch calves, hips, hamstrings, quads, ankles and focus on glutes strenght (main culprit, consider how much time we spend sitting).

I'm dealing with the same issue. The way it started with me was a second arthroscopic knee surgery I had on the same knee for a torn meniscus. Then out of nowhere I had a huge pain issue and was not able to go up or down a stair, it was extremely painful and I was not able to put weight on it. I had other knee surgeons and doctors tell me it will get better after a year or more, it has been about 3 years now... It got better, to the point stair was almost pain-free but I am not able to go run or even jog YET! The first surgery went well, recovery was very well, back at the Gym and jogging and running like normal but the 2nd surgery, I have no idea what ended up happening. MRIs show everything is fine, so why can't I put all my weight on it/ or jog or run like normal without a huge pain issue on my knee area. Leg extension, the most I can do is about 10-15lbs, it's that weak, very weak; Also why stairs is a bitch at times for me. I've been searching and searching on the internet for what I'm going through and Patellofemoral pain syndrome (PFPS) is the closest thing I have found relating to what I'm going through.

Damn 5 years now lol, I’m okay and at work wise all is normal, the pain has gotten WAY better, almost no pain at times. My knee still bothers me now and then though, shit that I will live with forever. I still like to ice my knee every once in a while. Since I posted what I said, the stairs have gotten WAY better, BUT running like a normal person NOPE! I can jog a bit but not run/ sprint. Years later I learned and know I favor my good leg, After that 2nd bullshit knee scope surgery I ended up with muscle atrophy for some strange reason. As of today my left leg muscle calf and quad are still smaller in size/ mass vs. my right leg quad and calf. I have not been able to get my leg muscles to match my other leg. Why back then going upstairs was an issue for me. Also kneeling on my bad knee still hurts, so I don’t ever kneel on that knee. Or use knee pads at work if I have to be on both my knees.
Looking back, I fucking regret ever having a fucking knee scope for a little small meniscus tear. The small little issue I was having with my knee was something I could live with and would have not had any of these fucking issues I have today. That’s life, wish I had a Time Machine and go back. That's my update, I'll probably one day, later later on visit a really good orthopedic specialist and check my knee situation but for now I'm okay, I work as normal, and walk normal thankfully lol, You can’t tell I have a knee issue, I don't let it bother me or complain about it. No one knows unless I talk about it.

I am 29 years old and I was diagnosed with Chondromalacia Patella and early onset arthritis by my doctor 2 years ago and his advice was to stop playing basketball and only do exercises that are low impact (swimming & biking). I don't want to give up my passion, but I know if I continue playing I will surely suffer later on in life. I know you don't have all the information, but if I follow the PT advice here and obviously consult a PT in person, do I stand a chance of improving the condition of my knee (going down stairs is really tough) and delaying the progression of my arthritis? A knee replacement may be in my future, but I want to delay that moment as long as possible, and being so young the thought of going through such a major surgery like that gives me anxiety. Any insight or advice you can give a young lad such as myself? I would greatly appreciate it. Thank you.

I was having a considerable amount (major) of crunching in my knees when I sat down low. I noticed my knees going outward when I sat down. The inner side of my left knee started to feel like something was wrong and slight pain occurred in that area very quickly and then went away when I sat down. I remembered when my physical therapist once wiggled my knee cap up and down and left and right to free it up when he first started me on therapy after being on crutches for a while. I had watched videos on it and remember doing it myself at home. So, this week I decided to do it again. What I found was my knee cap was stuck. I did a little up and down and left and right wiggling the first day to free it up slightly, and repeated for four more days only once a day. The creaking is oh so slight now, hardly noticeable at all. Basically my knee cap was stuck. My standing and walking is also better. When I sit down, my knees don’t go outward. They are lined up better. A couple more days ought to be enough.

I want to WALK as I crave the feeling it gives me but I know if I go back out immediately I will make things much worse. So I will do the exercises for the next 3 to 4 weeks and gradually start the walking routine again.
    AGAIN THESE EXERCISES WORK!!

Hi Bob & Brad! Great video, thanks! Hoping you guys still check out these comments! My question is, you mentioned most of the time the knee cap may be misaligned on the lateral side of the knee so you want to tape it and pull it towards the medial side. However, how do we know which way the knee cap is actually misaligned? Mine feels like its jutted more on the medial side but I'm hesitant to tape it as I'm just not sure.

Every since I was about 15, I am now 25, every once in a while, my knee cap pops or gets out of place and I will fall to the ground. I have not been too alarmed since it may happen once or twice a year or maybe less than five times a year randomly..I want to go see a doctor about this now, because it happened today. I'm just wondering if this is what I may have? I have never played sports or injured my knee in any other way than the times it's gotten out of place and I just fall or almost fall. Afterwards it hurts, I cry, and the pain goes away after a day or two...Thank you so much for sharing your knowledge, and wisdom with people.

My knees are so sensitive to touch, it feels like they are constantly grinding . The knee cap doesn't move & the walking has become a chore. This makes no sense to me. I tried several kinds of tape. The only time the tape actually holds the knee cap to the inside is when you are sitting and you first put it on. Once you stand on your leg, the force on the cap will push it back in the center. You can't even hold the cap over with your fingers while you stand, unless your leg goes backwards at the knee.

I've played tennis since high school and recently (about 4 months ago, right after I graduated from college), I started feeling some pain on the front of my right knee whenever I bent it while playing and also when I'm running. Two months ago I went to see a doctor about it and after physical evaluation and a couple of exams, I was told I have PFPS, and that I'd have to refrain from my physical activities and start PT right away. It's been 8 sessions now, but I still feel a lot of pain while performing the exercises, even though I've recovered some strength and my motion range is a lot better than when I started. I'm also worried because my right leg muscles (especially the quads) have atrophied a lot when compared to my left leg (it's over 2 inches shorter in diameter 7 inches above the knee cap) and I fear I might never be able to play tennis again because of this. I'd also like to know if I could stop going to the PT and do some exercises at home, since I've payed for 10 sessions and this can be really costly where I live (Brazil). My PT said I might need 30 sessions, if I'm lucky, but I'm afraid I might go broke first. haha
Thanks a lot for all the amazing information you guys provide on your videos.

+physicaltherapyvideo Thank you, Doctor. I've just talked to my PT and he scheduled me for an evaluation tomorrow, after my last session. Only then, he told me, he will be able to say whether I can continue the rehab program (if I can call it that way) at home or if I will need 10 more sessions. I've also asked him about knee braces and orthotics, but he said he wouldn't recommend them in my case, but he said that kinesio tapes (I guess that's what it's called) could come in handy. I watched another video of yours where you talk about this Mcconnell taping technique being applied on the knee (the "best self-treatment for chondromalacia" one) and I was wondering if that's the same as the kinesio tapes and if I could do that at home on my own, since my kneecap is also being pulled towards the outside of my knee. Once again, thank you very much! ;)

I've had intermittent dull knee pain for about the last 25 years. It was initially diagnosed as growing pains after x-rays and various tests turned up nothing. But as I got into my teens, nobody knew what was going on. I was diagnosed with patellofemoral pain syndrome after visiting the walk-in clinic when I was in quite a lot of pain and based on what you guys have said, it seems to fit. I don't really notice the pain gets better or worse acutely, any changes in the level of pain tend to happen over the course of hours, so running, walkning, getting up from sitting, going up stairs don't hurt immediately. But if I do them a lot, then I'll find that my knees feel very sore. The pain itself as I said is a dull pain, feels sort of generalized around the front of my knee, not like it's coming from any one specific area and it seems to go into the back of my patella and seems to go up my femur. When it's really bad I can feel the pain going all the way up into my hip. Which is the only part of it that doesn't seem typical. Anyways thanks for posting this, I thought it was very insightful. I'd never heard the term chondromalacia before and thought it was very interesting.

Hi, i also have chondromalacia and my orthopedic surgeon said that putting a brace on the knee can actually make it worse due to the brace pushing the knee therefor causing more pressure on the knee cap. the best thing to do is take cataflam and try stay away from exercises such as squats, lunges, cycling, running up and down hills, hiking. swimming is good but not breast stroke. basically anything putting pressure on your knee try not to do. Another thing that is good is going to a bio kinetics to try and strengthen the muscles around the knee to relieve the pressure off of that knee cap. listen i hope this helps, i'm not a doctor in any shape or form, this is just what i heard from my specialist.

What if the knee dislocated to the INSIDE? I dislocated my kneecap 5 times (over the course of 15 years) to the inside, and i had MRIs done and supposedly i have Grade 3/Grade 4 Chondromalacia (no ligament damage or anything else) but i have no pain on my knee AT ALL during everyday activities and im 30 years old. I got up stairs every day and i dont get any pain.
I get pain once every month in which i cannot straighten my leg because of the horrible stabbing pain behind my knee but it goes away after a day or 2. I cant run or take sharp turns obviously or else my kneecap can dislocate pretty easily.

This video helped me figure out I had Patellar tracking disorder (similar to Chondromalacia). They had me do physical therapy for it. Two months into physical therapy, I got right knee pain directly on my Osgood Schlatter ossicle. I taped over ossicle and pain subsided. I rested about a week and went back to exercising. The right knee pain kept coming back so I taped my ankles using kinesiology taping method for plantar fasciitis because I had so many lower leg problems such as shin splints, drop foot and plantar fascitis. With tape on, I noticed my knees felt weak and I could not brake well when walking. So while in a car, I tested out my leg strength by pushing down on my legs. I had trouble pushing down on my right leg. That's when I realized Ossicle Schlatter ossicle was weakening my leg strength. Later on I realized I wasn't using my right leg to get up from ground, couch, or chair. My left leg was doing all work. I could not dip or hold my weight on squats and single-leg squats with my right leg. When throwing out heavy stuff like trash, I could not have the heavy stuff on right side of my body because my right leg couldn't support weight. So I brought electric tape and taped over ossicle (not above it but over it) using a taping method called Crystal Palace Wrap (youtube has plenty of videos), I immediately got some leg strength back with electric tape on (it has to be electric because other tapes are too thick and heavy and will hurt your patellar tendon). I could get off ground, I could dip and hold some body weight on squats, I could hold weight of stuff like trashbag a little longer on right side of my body. I told Orthopedic Doctor this and even brought electrical tape with me to display it to her. She had me do Ultra sound guided knee injection on ossicle. A few weeks later, she approved surgery for Osgood Schlatter. It's fixed my Chondromalacia / Patellar Tracking Disorder along with my fatpad impingement.

+PMcFarland Oh man, I feel you. I've had the problem since I was 12-13, and so many doctors have told me I just needed to lose weight (I was not overweight when I was 13!!) or do more exercise. Because it's just that easy, I guess.

Been looking for a video by you both on Posteria pelvic tilt. I am female,5 ft 8 inches tall and have a flat sacrum, short pelvis and long spine. I am now 82 and after a lifetime of standing straight and walking a lot, I now bend forward from my lower lumbar. I can’t walk any distance anymore. Recently I found a bump in my spine which pushes my spine to the rear. I found I can pull up with my upper back and feel the bump disappear. I can then walk as long as it stays in place. I was wondering if you could do a video on this which I think is posterior pelvic tilt but since it’s a self diagnosis, I wouldn’t bet on it.

I have been diagnosed with this syndrome 4 years ago, and with various methods of treatment (icing, rest, otc pain relief, j tracking braces) nothing has helped eased my pain. Is surgery a potential long term option?

I put leukotape on by keeping my leg straight, pushing my kneecap over, and taping from the outer edge down over the inside of my knee like in the video but the second I bend my leg it rips right off. Am I putting it on wrong?

Hi, hope you don't mind me commenting! From an x ray and an mri scan, my physio says I have minor wear & tear behind the kneecap, the beginnings of osteoarthritis. I presume this is a separate condition? My physio says my prognosis is a progressive condition but that as it is now it is 'as normal a knee as is possible without actually being normal!' Hes told me to get tough with the strengthening of the knee. Ive read that the condition of arthritis doesn't necessarily get worse and wondered what your thoughts are? Thanks!

would it be possible for the kneecap to be tracking incorrectly but pulling more to the inside of the leg? I feel the pain more on the inside edge of the kneecap than the outside. I'm a regular road cyclist but recently I am finding my inner quad would be tight/sore after some rides. thanks

Im an avid runner and a year ago I fell really hard directly on my kneecap and it has never been the same ever since. Could that cause this condition? Also, if my pain is on the medial side and under the kneecap, would that mean that the kneecap is also tracking too far to the medial side? Not sure if that can be determined by where the pain is. Thank you very much for the video!

were messing around with this for school clinic, and found a paper stating that Mc Murrays has 86% false positives for chondromalacia when surgery intervention was applied... can't find the paper again now thou, but clinical thoughts?

it was on pubmed, surely a visual inspection can not always exclude early stage onset of the disorder, but the % was so damn high.
