I Mean you have too do residency for 2-4 year which start at 60k a year pyg1 of 60k to pyg5 of 87k depending on the specialty. that is highly enough to pay back at 120-150k debt(domestic student) even with the interest rate given they don't spend their money like it no tomorrow. This can probably be a justifiable debts that I would feel proud to pay back.
