Does position whether head down or up affect the spread? Also for unilateral hip surgery, keep the patient on side till drug fix is accepted?

There is an increased risk of an epidural hematoma when advancing an epidural catheter from the caudal to the thoracic region. Wouldn't it be safer to use usg thoracic epidural?
