I continue to watch these instructional videos because I think I must be doing something wrong. This is a very frustrating block, in my opinion, for providing pain relief for TKA.
Great instructional video. I have done these blocks repeatedly for TKA's over the past 4-5 years and my technique is exactly like yours, sometimes I even make certain I have spread on both sides of SFA in the canal. I always use a minimum of 30 ml local volume and NO ONE CAN CONVINCE ME THIS BLOCK IS NEARLY AS EFFECTIVE IN PROVIDING POST OP ANALGESIA FOR TKA, compared to the old fashion 3 in one block. Does anyone agree with me?
Small tip: Add dexmedetomidine at a concentration of 2.5mcg/ml (50mcg/20ml of local anesthetic solution) and dexamethasone at a concentration of 0.2mg/ml (4mg/20ml of local anesthetic solution) to increase the duration of the block by over 100% (from 6-12h to 24-36h).
