The people who assisted him must be airline ground employees. The upgrade is not unlikely if this was a fully booked flight. The airlines must have given his assigned seat to a waitlisted passenger. They moved him to an available seat, which happened to be in the premium economy class. There is a higher chance of getting an upgrade if the plane is full and you have a frequent flyers membership. Otherwise, it would just depend on pure luck.
