Né dans le camp de Khan Yunis en 1962, il y étudie puis rejoint l'Université islamique de Gaza, où il devient secrétaire du Département des Sports, puis de l'Union des étudiants.
Emprisonné une première fois en 1982, il est placé à l'isolement.
Après sa libération en 1985, il crée al-Majd, une cellule de l'Association des Frères Palestiniens chargée de traquer les collaborateurs des forces d'occupation.
Il a été arrêté une seconde fois en 1989 et condamné à 426 ans de prison.
En octobre 2011, Yahya al-Sinwar faisait partie du groupe de détenus libérés à la suite de la signature d'un accord d'échange de prisonniers avec Israël.
Entre 1989 et 2011, il a dirigé le Hamas dans les prisons israéliennes.
Lorsqu'il a été libéré en 2011, il avait purgé vingt-deux ans de sa peine. Il est ensuite élu membre du Bureau politique et devient le coordinateur entre les ailes politique et militaire du mouvement.
En février 2017, il a été élu chef du Bureau politique du Hamas dans la bande de Gaza. Il considère l’union avec le Fatah comme une option stratégique et a donc poussé à un accord pour parvenir à un accord au Caire en octobre 2017.
Si les dirigeants du Hamas ont unanimement souligné son respect des principes de consultation, certains ont néanmoins affirmé que Sinwar a toujours su imposer ses choix au reste du mouvement par des moyens qui incluent parfois des menaces.
Il entretient de bonnes relations avec l’Égypte, qui le considère comme un véritable leader national palestinien donnant la priorité aux considérations nationales plutôt qu’aux affiliations partisanes et idéologiques.
En 2021, al-Sinwar a été réélu à la tête de la direction de Gaza. Sa popularité a augmenté lors de l’attaque de Gaza en 2021, et il jouit désormais d’une véritable stature nationaliste parmi tous les Palestiniens. »
