My last playthrough, I lingered in Chapter 2 for months, doing everything BUT visit the Downes Ranch. I wanted to prolong Arthur's health as long as possible. Then I spent about a week, real time, sniping Downes from the road to see if I could just rob him and settle the debt without getting sick. I would take his body into the woods behind his house, but soon another Downes would appear in the garden. So I sniped him and dropped his body on top of Downes #1. I went through about a dozen Downses before giving up on the game ever letting me rescue Arthur.
I guess there is more background story behind Arthur's health. Spending time in chapter 2 long enough and you realize that Arthur has something wrong anyways. He wheezes and coughs from the very start. Mainly in close quarters like dressing rooms and when he goes up north he will cough more and more the longer he is up there.
Not saying people shouldn't do it or anything but I wonder why people always say that they tried somehow to save Arthur by killing Downes early or drinking lots of tonics. I get that we love Arthur and don't want to see him get sick but RDR2 isn't an RPG neither, of course there's no way we could change such an important plot point like Arthur getting TB inspiring him to gain redemption. Unless most of the time it's said is meant to be a joke.

It's an act of profound empathy. For my part, I have additional reasons: I have severe asthma, so Arthur's struggles to breathe as the game (and his disease) progresses, especially the wheezing and the weakness, feel all too real to me. And I first picked up this game in the first summer of the pandemic, so, given my particular vulnerability to COVID, watching Arthur die of a respiratory illness while people all around me were dying of a respiratory illness that I was (and still am) desperately trying NOT to die from . . . That was hard. Arthur's death was very, very hard on me. So in my long Chapter 2, I wasn't so much trying to "save" Arthur as I was delaying the inevitable pain of watching him die.
i mean technicaly you "can save him" if you literaly dont touch herr strauss mission and he'll leav camp by him self with you never even meeting the downes family but you get sick reguardless :C but hey have some satisfaction knowing if your quick enuff in the final mission you can kill micha in the good and bad ending (in the good ending if you spin around at the right moment and activate dead eye you can kill him) (if your quick and cunning enuff to block and attack micha you can kill him in the knife fight forcing the game into a soft lock where your free to explore but no missions are available thats what happend to me at least)
To be fair i also spent a long time trying to cure him too. I really tried my best with it but also failed. The best cure is not progressing the story.
I think you can on a very technical level. It won't be fully cured by appearance nor story wise nor Micah calling you black lung, but you can remove the negative effects like health core decay (I think??) by using certain trinkets and talismans. But amazing video as always.
Yeah there's a mission called "Archeology for Beginners" where if you do it properly you'll get a talisman that negates all core drains you get from TB
There is a way appearance wise. The game for some reason will set Arthur's appearance as underweight even if he's morbidly obese. My strat is eating two meals at a saloon and then sleeping or doing anything that changes weight like fast traveling, which will alter his appearance to the correct weight. Unfortunately, he still looks like death, and if you alter his appearance by changing clothes or cutting hair, it'll fix the appearance to underweight. My other suggestion is just don't do A Fork In The Road until you've done any side content you plan on doing, since you'll only be hit with -5% drain to all cores, and Arthur won't cough in free mode.

In one of the missions with the Indian tribe leader he tells you to get something from a military base without killing anyone it is very hard and it accepts it if you don’t do it without killing anyone which made me think it was impossible not to kill anyone but if you do it right he gives you a talisman that gets rid of all the negative affects of tuberculosis but sadly Arthur still dies at the end
I vaguely remember this mission. I'm guessing you have to try to sneak as close as you can, then just run like a madman using all the health items at your disposal to survive.
The cure for TB wasn't discovered at the time the action takes place. However the ingredient necessary to make it were on the planet, so if someone were to mix them just right, the cure would have been obtained. The question is: are those ingredients pickable items in the game or not?

Just started RDR2. I'm nearly out of story missions in Ch2 and that dirty is fast approaching. I'm riding all over, making discoveries with Arthur, giving him the long haul.

Oh there’s an easy way to cure him. You get a mod that makes Arthur have his chapter 1 model. You get another mod that removes the coughing (obv wont remove the cutscene coughs) you then use the Rampage mod while in the epilogue and change Johns model to Arthurs. He will act exactly like Arthur and he even has voice likes in blackwater and new austin. You get a nice house for him and a wife and a son (abigail and jack lol) and you just pretend John saved Arthur instead of the other way around
@Azril Iskandar There's a theory that Arthur and Abigail were together before and during John's time with her. The theory proclaims that is why Arthur and John don't like each other for a little while and also why Arthur helps John at the end. Making sure Abigail and Jack (who could be Arthur's son in this theory) live a long life, safe with someone who can protect them.
There's an opportunity to interact with Downes in Valentine prior to the debt collections mission. If you do that, he coughs in your face. There's also a hunting mission you can do before collecting from Downes where Arthur has a cough.
The only way Arthur's could have possibly survived TB is if he had left the gang and stopped stressing his mind and body. He was young and healthy enough to have survived it. But what a boring game that would be... The TB and the Death of Arthur is unavoidable its part of the main storyline and it's one of the things that makes this game so brilliant and so loved.

i was really hoping that R* will release a DLC titled "The Good Ending(?)" where Dr. Marko Dragic found Arthur's body and do a lung transplant and uses the electric machine to revive Arthur or incubate Arthur in a cryotube. Few years later, Arthur wakes up and seeks revenge on Micah and Dutch and Arthur dies again because of organ failure(new lungs).

It's built in story-wise thing. Programmed. Even if you mod the game you beating Downes in hazmat suit, or even if you mod the game to beat and win that mission with being mile away from Downes, you'll get it. It's Programmed for Arthur to contract TB. Only fully modded game can cure Arthur.
This mission was a big turning point for Arthur. Mr Downes spit directly to his mouth which gave him tuberculosis. The game will automatically remove your facial wear. So it’s impossible for Arthur to prevent tuberculosis

The doctor said he must rest so I kept him in Saint Denis for a couple of weeks just eating proper food and sleeping alot and He got a little better but the disease won't be cured

It says in player wellbeing the "Sickness can not easily be cured" which implies that it can be cured.

I never went to the place where he got tuberculosis I just ignore that one part of the quest so he isn’t sick

It's built in story-wise thing. Programmed. Even if you mod the game you beating Downes in hazmat suit, or even if you mod the game to beat and win that mission with being mile away from Downes, you'll get it. It's Programmed for Arthur to contract TB.
Only fully modded game can cure Arthur.

This mission was a big turning point for Arthur. Mr Downes spit directly to his mouth which gave him tuberculosis. The game will automatically remove your facial wear. So it’s impossible for Arthur to prevent tuberculosis
