You first apply the numbing cream, after the client is numb you should clean the surface and map the area. The numbing cream will wipe away the mapping if it were applied first.
It shouldn't hurt at all. I'm a microblading artist and have had clients fall asleep. Some artists don't like to use numbing cream, I have never done it that way. I have had other say it was a bit uncomfortable, but I feel that may be because you can hear the blade. The process in this video is very different then how I was taught.

I agree, this does look painful. They did use a lot of numbing cream but it is very different then how I was taught. It should not be painful
