980username Make sure you're allotting all the values as they are shown in the video. I use this build as my main template for all my other cosplay build and I have no trouble recreating it following this same video
I've noticed that Dark Souls 3 is just weird like that. From what I can tell, the slider values work slightly differently across consoles and maybe even across different updates possibly. What you describe now has actually happened to me before when recreating other characters based on slider values. Best advice I can give you right now is what I do myself when the slider values don't work, just tinker with them until you get something good. Best of luck with that, hope you manage to work it out

If I actually had a pc or Xbox copy of DS3 I might've actually tried to make a sliders showcase for that variety of values, or in the least try to figure out just how different the slider values can get across consoles.
Resolution might have a big effect on it. I was playing DS3 on the lowest resolution (PC) and created many characters with tattoos, ans when I increased the resolution, the tattoos became smaller on the face. Might be the same with the character model itself.
them’s the sellsword gauntlets from the sellsword set. You either have them if you start the game as a sellsword, or you can pick em up on the Road of Sacrifice
The weapons are the Greatsword UG in the right hand and the Cestus in the left handThe armor pieces are Abyss Watcher's top, Sellsword Gauntlets, and the Alva Leggins (although the Abyss Watcher leggings look nice as well)

Most of the numbers worked for me except for the scar, it ended up in his hairline

It’s the armor you can buy after killing the Abyss Watchers, I believe it’s called the Farron Watchdog armor
Lotric Knight leggins are better for Black Swordsman armour.
Those are wayyy too fancy for Guts. That'd be something that would look good for a Griffith build though
Nevertheless, they look more like his leggins. And they do not make his legs thinner. Outrider knight armour looks more like Griffith's armour.
