you have the set of judgment set up so that those on the top are severely punished for all actions. this would force CEOs and Government officials very engaged in what is going on in their institutions and rooting out issues before they happen, because if an issue happens those in power will be punished in a similar way as the one that did the crime

Fair point. I am not sure, however, that the threat of punishment is ultimately a deterrent. This police officer knew that, if caught, his life would be ruined. He did it any way. I think accountability from friends and colleagues, before the fact, is ultimately far more likely to produce results.
