    Survived a Type A Dissection Jan 7 2020. I month in the hospital. Fluid trapped in my left lung, PFO opened up had to have a double plug installed. Doing fine almost 2 years ago now. UNC Chapel Hill Medical Center saved my life.

    December 2, 2019 ascending aortic aneurysm with rupture and dissection. They found an abdominal one as well but was too risky. Going to Cardiologist at 10am this morning. After going through the first one without warning I am terrified because my right side of stomach and lower back has been hurting for a while and sadly this is the first appointment I could get. During the surgery they took a huge chunk of muscle and cartilage under my right shoulder almost 2 to 2.5 inches deep im guessing. I was on a wound vacuum for two months with 30 staples in the shoulder area. It looked like my twin nephews were there having fun with staple gun. I have lost a lot of feeling in right hand, mainly thumb and pointer finger. My right shoulder and elbow have random sharp pains and dull aches. I also had veins stripped from my right leg near the calves. I always some plaque psoriatic on elbows, sides, neck, and on my head. I now have psoriatic arthritis as well.
    I'm not complaining because I'm still here and I promised my Dad I would take care of my mom literally 15 minutes before he left his cancer behind and went home pain free. Yes, I am a Mamma's boy and very proud of it. My parents are my best friends, I live back at the house I grew up in with my mother who is 75 and I'll be 45 june 20th. When dad got sick I was able to be here so he didnt have to go to hospice, my mom couldn't do it alone because he was 6'3" as am I so he was able to be where he wanted to be and well taken care of. Two and a half years ago I bassically died then 3 months to the day I was back at work as a Food Safety Specialist. That lasted until a seizure in company car 2 months later and have been out since. I get very depressed because I dont get to help my mom financially even though i took short and long term disability with my company insurance which declined me the first time and waiting next week for the second their decision again. I miss working so much to where I wasn't always honest with my dr's about little things and trying to get back to work. My rehab was nonexistent due to Covid. Im worried about the abdominal one but other than that I am thankful, eternally grateful, and happy to be alive so me and mom can take care of eachother. God has a plan and I have faith in his plan! I do get down, depressed, angry because things I can't do anymore but there are people a lot worse off than me. I'm still able to watch my Chicago Cubs play and my mom's team, the Braves, play just about everyday with her and my huge ferocious pug Lemmy. Before all this we would drive down to Atlanta for a lot of games and fly to Chicago as well for weekend games 3 to 5 times a season. I'm just happy spending time with Mom, my nieces, nephews, brothers, and friends. I'm even happy to see my exwife. She lost me by playing in the grass on the other side of the fence at her work! I have forgiven her and even though she broke my heart and trust not just for her I asked for forgiveness from her because we were a team and I failed her at some point for that to happen. We are lucky to still love and care for eachother and even made friends with her fiancée. Again, im just thankful to be alive and above ground so maybe one day another beautiful heart will see me on my side of the fence.

    Great point about the Perfusionist fixated on the line pressure. The ability to “see the big picture” is difficult when he doesn’t have the proper information flow. However, could this event have been prevented by the anesthesiologists TEE studies prior to cannulation? Indeed, aortic dissection in this scenario probably happened upon initiation of bypass due to the aortic cannula positioned in the false lumen. But working through the problem, in the Perfusionist’s brain, starts with line pressure, because it is more common to have faulty disposables, transducers, etc than an actual dissection. Great simulation!

    How do u guys check for arterial cannulation site? I see u check for pressure swing, and why do you test running volume for a certain flow then focus on the pressure back with low flow with the higher pressure, i think its should already tell u that the cannulation site might be in false lumen or might be clamp or wrong position, i think rather going on for bypass u already should tell ur surgeon that the arterial line were not good position rather to take a risk to go on and might worsing the dissection more, just my concern about it

    why the anaesthetist is always in so hurry to switch off ventilation.... ideally he should wait for perfusionist to announce that he is on full flows and oxygenation is good....

    I have a triple A and will be having surgery soon. I'm scared shitless.

      you won't be going on Bypass for a Triple A.

      @Beer Monster.....I have an abdominal aortic aneurysm, a thoracic aortic aneurysm and a tear in my aorta. I was diagnosed on 11/28/17 and I am currently at about a 5.3cm. I had surgery scheduled in March, 2018 and I cancelled. I have even had a 2nd opinion. I cannot find a Doctor than shows one ounce of compassion. This operation presents severe outcomes (paralyzation of legs, loss of all kidney function, severe stroke, massive heart attack and brain injury). I am also scared shitless (beyond belief). And I have to have this surgery fairly soon. Have you had your surgery and how are you doing? I hope everything went well for you.
