  • - Mechanism of Speech
    - Types of Sound
    - Prosthetic factors affecting speech:
    1- Denture base thickness:
Well adapted denture (1 mm thickness) not greatly affect speech.
    ↓ thickness → Whistling (D pronounced T).
↑ thickness → anterior palatal part → Lisping.
    → posterior palatal border → vowels (E-I)& consonants (K-G).
→ Lingual flange → (Cramped tongue) Lisping.
2- Extension of the denture base:
Good retention& stability → Aid to stabilize during articulation of sound.
→ Tongue and lips aren’t busy to retain the D.
Proper extension → ↑ retention& stability → tongue rest during speech.
→ ↓ interference with mm. (lips) during speech.
    3- Polished Surface:
Reproduction of (incisive papilla & rugea area) → correct ant. palatal s.
    - Denture relations:
    1- Occlusal plane:
Too ↑ occ. plane → tongue higher than teeth → lisping.
    Too ↓ occ. plane → wrong lip & tongue position with teeth → (V pronounced F) .
2- Vertical dimension of occlusion (VDO):
Too ↑ VDO → Clicking, Whistling, Defect (Ch-C-J), (Th prononced T).
Too ↓ VDO → Lisping (S pronounced Ch).
Clinical app. of M & S sounds in Clinical determination of VDO.
    3- Teeth arrangements:
Width of dental arch → narrow arch → cramped tongue → affect consonants (T-D-N-K-C) → (set the teeth in neutral zone).
