Also. If you are referring to MDS you should have used the right coding numbers
Toileting does not include toilet transfer. They are separate.
Every Nurse should have this training
Significant change takes the place of other assessments ehen there is a sig change and Is a comprehensive assessment.
Annual is due 366 days after the ARD of the admission assessment.
Obra assessments need to be done every ,92 days equals admission, quarterly assessment.

Hi Jen. i am new with MDS and is required to open an assessment after each admission. I find it very hard to figure out how to choose from this question (1). is this a CMS assessment? ( 2). Optional state Assessment A0300A (3). Optional state assessment type A0300B. please I need you help. I read the RAI manual but did not really understand it.
Hi Mercy! Most of the MDS assessments are CMS required assessments. The only exception would be Managed insurance assessments should not be sent to CMS. The Optional State assessment is determined by the state you live in. You may need to reach out to your state RAI Coordinator. Since the RAI manual was not updated you will not find the answer to your question in the manual. IJust remember everyone regardless of payor type is required to follow OBRA requirements. This means everyone need an Admission assessment, Quarter assessments, Annual, and Significant change assessments. Tracking records such as the entry and discharge MDS assessments are required for all residents. I hope this helped. Please let me know if you need further assistance. Have an amazing evening!
