Is this achievable to a middle level player
For this part of the song, this probably is achievable in a short amount of time with enough arpeggios practice, but the whole song overall would probably be fairly challenging, but if one is motivated enough I could definitely see an intermediate player potentially pull it off although it would take a while for an intermediate player, so its more so a matter of time and dedication more than actual ability as the main challenge is the arpeggios. If you can get those down, you can probably do the whole song (P.S there's no shame in simplifying a song and doing it your own way, as long as it sounds good )

What software do you use to make such good midi visuals like this?
I use a software called SeeMusic which has a lot of cool features and it's also free but you need a subscription to get rid of a watermark but there's also another one called Embers which Sheet Music Boss use and is also free but I feel like with SeeMusic you can do a lot more with the texture of the notes, so I'd suggest looking into both to see what you like and prioritise :D
