I can’t believe that this treatment is not more well known and I think there must be lots of people sitting about waiting for things to get better and then needing more surgery, or living with results they are unhappy with. This is such a good solution!

    Hello Doctor.. I was told that compression and more lymphatic drainage massages help with the fibrosis. However, I noticed fibrosis within the first week and just had 10 massages post- op. What do you suggest?

      Hi Dr Chi - yes MLD and compression do help, and sometimes that's all that is needed. However I now like to intervene early, as soon as I am aware of it, with recombinant collagenase enzyme injections. I just find it so rapidly effective that it saves everyone a lot of time and trouble. Patients can become quite upset by the feeling and appearance of fibrosis and I think it's good to nip such feelings in the bud as soon as possible!

    i wore a sports bra that was far too tight while sleeping for one night (day 6 post op. probably had it on for ~15 hours) and i definitely noticed a changed shape when i took it off :( wish i found this video beforehand! do you think i messed up my results or do they still have time to go back to a more well rounded shape? i’m still on day 7

      Hey, thanks for your message. It's impossible to say but I'd say that the best thing you can do now is to go back to a proper postoperative support bra. Please contact your surgeon and take their advice on aftercare. The shape should revert but I don't know exactly what surgery you've had so it's difficult for me to comment - sorry!

      Yes you will find some surgeons who would be willing to perform this under local anaesthesia, usually with at least some sedation, depending on the extent of surgery in your particular case. However at the moment I tend to perform cases like this under general anaesthetic because I think it is a bit too much for a patient to go through awake, and general anaesthetic has such a good safety profile.

      It can be, but generally not in my practice. I only place implants under muscle if there isn't enough fat/breast in the upper pole to cover it adequately - this normally means a pinch thickness of less than 2cm. However in tuberous cases, since I'm fat grafting, I can simply graft the upper pole to make sure there is enough thickness. Placing the implant above the muscle has the additional advantage of expanding the lower pole more effectively, which can otherwise be a bit of a problem with tuberous breast cases.

    You are so right. I'm 8 months post-op, MMO no implants or BBL i had breast reduction and lift. Ab repair/TT, lipo 360. I struggled for a fair few days after surgery. Flying back, manoeuvring myself at home, showering and sitting comfortably at work. Even though everything went swimmingly well, i can honestly say to all reading and watching this.. DONT DO IT ALL TOGETHER.. no way, hosay. I'm so glad to see how open and honest you are. So Thank you. 😊

      Ah thanks - it's good to hear this resonates with your experience. I do feel pretty protective about my patients - surgeons get so used to being surrounded by surgery that I think it's possible to get tunnel vision, so my sense check is always: would I be happy if some other surgeon did this to my partner/spouse/parent... If the answer is no, then I'm not going to be that guy for someone else's partner/spouse/parent...!

      Ah - that’s not so uncommon, but not very nice after a muscle repair. Always speak to your surgeons team, but I’d think about the pain killers you’re using first of all. Could you cut back on the opiates? They constipate you and I find often make the postop period more UNcomfortable, because the constipation is more uncomfortable, so then you take more and it gets worse. Try simple analgesics if you can. Also I tell my patients to drink plenty of fluid and to think about stool softeners too. Best of luck with your recovery.

      Hi - yes after the operation exactly as shown here, breast feeding shouldn't be affected. However there are different types of breast lift which certainly could impact breast feeding, so if/when you consult with a surgeon you should ask specifically whether it would be affected in your case.     I had surgery 40 years ago, just got implants out last and look worse then if never bothered. I ended up having nipples so tender they hurt to be touched then slowly just went numb and I can barely feel anything. I hope times have changed and not so much nerve damage now.

      Wow that's sad, I'm sorry to hear that. Thankfully that's not normal at all these days (I don't even know that it would have been normal 40 years ago?). Nerve pain is exceptionally unusual - I can't think of any patient who has developed this - and there are options for gradual improvment in your natural breast tissue at each implant exchange so that when they finally come out, you are left looking pretty much as you did when they were in. But this is not standard surgery and it's important to research your options very carefully. Thank you for sharing this 🙏

      @@eterno360 I don't remember the doc saying the nerve problems would happen but he said to make the breasts more round he would have to cut into the tissue as if cutting out pieces of a pie, if that makes sense, and then put the implant in that. I just assumed that all that cutting is what damaged the nerves. I think that is also why I look so stretched and deflated now. I went from about an A cup to a C (Canadian/USA sizes) and between the length of time I had implants and natural aging which can result in sagging, that is what I am left with. I would get a lift at the very least if didn't have to pay for it. The explant was free since implants were leaking and also causing me alot of pain especially lying on my back, they just flopped to the side. now they REALLY flop to the side when on back. Seriously, just wish I would have left them alone but I get why women want the surgery. Back then there was no online pictures like there are now to know I wasn't alone and such a freak.

      It's a great question, and a long answer. But there isn't really much you can do as the patient. Statistically it is something that becomes increasingly likely the longer the implants have been in. However there are definitely things that the surgeon can do at the time of surgery to reduce the risk, or rather prolong how long it takes for capsular contracture to develop. So the best thing you can do as a patient is ask your surgeon what they know about the causes of this, and what they do during surgery to avoid it. They should be able to list a whole load of measures they take, and if they can't maybe continue looking around...

      @@N0N4M30 Ok - they should be telling you most of these: intravenous antibiotics before surgery. Avoiding periareolar incisions. Using nipple shields. Ensuring atraumatic dissection. Maintaining careful haemostasis. No dissection of breast itself, just the planes below it. Using a dual-plane pocket (more controversial). Irrigating the pocket with an antibiotic solution. Reducing skin-implant contact eg with a funnel. Limiting implant exposure - ie open it immediately before you use it and close the pocket straight away. Changing gloves and instruments before handling the implant. Avoiding drainage tubes. Closing wounds in layers. Using post-surgery antibiotics 🎉

    I have skin excess no muscle separation (it appears to me) 59 yrs old and had 3 kids. Non smoker always, Now just ready to have my tummy tuck on 1/29/24. Been working out on treadmill and weights and have developed some toning to my body but the skin on my belly is full of stretch marks from belly button down, such a downer to see. Thanks for the information here! It helped.

      I'd recommend Micropore tape by 3M. You can get it easily on Amazon (and it lasts at least a few days - you can shower with it and dry it with a hairdryer afterwards)

      The fluctuating bloating is so normal - don’t let it get you down! You will eventually settle down to the ‘not swollen’ state permanently, but it can take months (even 3-6 months) for the end of day swelling to go away. It can be a sign you need to be slightly less active or keep going with compression or MLD a bit longer - check with your surgeon for advice tailored to you and your surgery.     I know this is quite an old video but I’d like to know about different body parts. So let’s say I do arm lipo in November can I do 360 waist lipo in December or January. Would it be safe and i believe both are awake procedures.

      Hi - yes from a safety perspective you could have multiple anaesthetics in a relatively short period of time, such as monthly. For example in urgent hospital situations, we may operate on patients multiple times a month, or even multiple times a week, without any particular consequence of the multiple anaesthetics as long as each is performed safely. In the context of an aesthetic procedure it's therefore not the anaesthetics that limit us, but rather the need to wait until the results of the last operation are known before performing the next one. Usually I recommend waiting a minimum of 3 months, and it's often more like 6 months in practice. I hope that answers your question!

    I’m 4 days post op today. I’m feeling pretty good. I’ve been taking Arnica Pellets under my tongue 4-5 times a day plus Turmeric in morning and turmeric/cardamom in the evening. Im not swallow at all. I had MM with Lipo on my sides and fat graphing in my chest. I had to go to Drs on Friday because my R bulb won’t stay deflating but There working ok. I was able to see the results and they look good.

    Really disappointed to hear that Earfold is no longer available ☹️ It seemed like the perfect procedure to fix an insecurity of mine. Do you know if there is an alternative to Pinnaplasty that works like Earfold that could be used instead? Thinking of having this procedure in the next 2-5 years so I can wait haha. Do you think it might be available by then?

      U can try incisionless otoplasty. No surgery needed it is using suture technique. Less costing, and less hurt

      Hi! You should contact your surgeon as they will have their own protocol. For my patients, I get them to come back in and drain it through a needle attached to suction, and we repeat this once a week until there is no more seroma. Between drainages it's really important to maintain good compression. Some surgeons inject dilute steroid or caustic substances called sclerosants into the space to make it stick down and prevent seroma recurrence, but I haven't found this necessary in my practice.
